Job Description

MDS Coordinator (RN or LPN)
Holly Creek 

Caseload: 24 residents
On-Call: Rotation required

Compensation:

  • RN: $82,300 – $99,600/year
  • LPN: $72,800 – $86,600/year

Final pay determined based on licensure and experience.

About This Role

This MDS Coordinator position offers a focused caseload of just 24 residents, allowing you to build deeper relationships with residents and truly know the individuals in your care. You'll partner closely with the interdisciplinary team to coordinate accurate, timely assessments and care plans, with the added responsibility of an on-call rotation.

Position Summary

The MDS Coordinator teams with multiple departments to coordinate the development, completion, and ongoing updates of resident assessments and care plans, in accordance with state requirements and the policies and goals of Christian Living Communities. This role monitors and guides the completion of PPS, OBRA, RAPs, and Triggers assessments, steps into direct nursing care when needed, and participates in surveys and inspections by authorized government agencies.

Essential Duties

  • Oversees and evaluates the gathering of information for assessments and care plans; schedules and facilitates care plan conferences
  • Monitors and guides the completion of PPS, OBRA assessments, RAPs, and Triggers
  • Maintains and periodically updates written policies and procedures governing the development, use, and implementation of the resident assessment (MDS) and care plan
  • Transmits completed MDS to appropriate parties on a weekly basis; maintains 15 months of completed MDS on all residents for surveyors
  • Assesses prospective residents alongside other team members as part of an interdisciplinary team
  • Performs the function of a nurse when needed
  • Completes administrative duties such as medical forms, reports, evaluations, and studies as necessary
  • Develops and maintains an ongoing quality assurance program for resident assessments and care plans
  • Participates in on-site surveys conducted by authorized government agencies
  • Participates in on-call rotation
  • Other duties as assigned

Qualifications

  • Active RN or LPN license in good standing
  • Prior MDS coordination experience strongly preferred
  • Working knowledge of PPS, OBRA, RAPs, and Triggers assessment processes
  • Strong organizational skills and attention to detail
  • Ability to collaborate effectively across departments and disciplines
  • Willingness to participate in on-call rotation

Qualifications

Basic Qualifications & Experience

  • Registered Nurse, with a current, unencumbered  license in the state of operation
  • At least two (2) years of managerial experiences in a hospital or nursing care preferred.
  • Able to demonstrate the knowledge and skills necessary to prove care appropriate to the age-related needs of  the residents served
  • Schedule flexibility, with ability to be rotating managerial on-call.
  • Able to problem solve independently when warranted, demonstrates knowledge of medical practices and procedures as well as laws, regulations, and nursing care communities
  • Basic knowledge with office software applications including Word, Outlook and Excel.
  • Must be able to read, write and speak the English language.
  • Must possess excellent customer service skills.

Working Conditions / Physical Requirements

  • Light physical effort required by handling objects up to 20 pounds occasionally and/or up to 10 pounds frequently.
  • Sits, stands, bends and moves intermittently during working hours.
  • Is subject to frequent interruptions.
  • Communicates and interacts with residents, family members, visitors frequently.
