ABOUT THE ROLE:

The MDS Assistant provides clerical and administrative support to the MDS/Care Plan office in a skilled nursing facility. This role requires an active CNA certification and combines clerical accuracy with working knowledge of resident care documentation, managed care processes, and care conference coordination. The MDS Assistant serves as a liaison between nursing, therapy, business office, residents, and responsible parties, while also acting as the facility's CNA documentation champion to support accurate point-of-care charting.

In this role you will:

  • Offer general clerical and office support for the MDS office and includes the following: 

Managed Care & Insurance Coordination

  • Track and process incoming managed care insurance updates, authorizations, and notifications
  • Communicate coverage changes, authorization dates, and benefit updates to the MDS Coordinator and interdisciplinary team
  • Maintain accurate logs of insurance authorization periods and flag upcoming expirations

    Care Plan Coordination & Scheduling

  • Schedule care plan/care conference meetings and coordinate calendars with residents, responsible parties, and interdisciplinary team members (nursing, therapy, social services, dietary, activities)
  • Prepare and distribute care plan attendance sheets and sign-in documentation
  • Send meeting notifications/reminders to families and responsible parties via phone, mail, or email
  • Maintain accurate records of care conference attendance and follow-up items
  • Assist in preparing care plan meeting materials and documentation packets

    CNA Documentation Champion

  • Serve as the facility "champion" for CNA point-of-care documentation accuracy and completion
  • Provide ongoing training, coaching, and refreshers to CNAs on proper documentation practices (ADLs, restorative care, behaviors, skin checks, etc.)
  • Monitor documentation compliance reports and follow up with staff/supervisors on missed or late entries
  • Assist in onboarding new CNAs on documentation systems and expectations
  • Support audits related to CNA documentation for survey readiness

WE ARE EXCITED ABOUT YOU BECAUSE YOU HAVE…

  • Active, unencumbered CNA (Certified Nursing Assistant) certification in good standing
  • Prior experience in a skilled nursing or long-term care setting preferred
  • Familiarity with MDS/RAI process, care planning, and electronic health record (EHR) systems (e.g., Point-Click-Care, Matrixcare) preferred
  • Strong clerical/administrative skills: typing, filing, scheduling, data entry
  • Excellent organizational skills and attention to detail
  • Strong verbal and written communication skills for interacting with residents, families, and interdisciplinary team members
  • Ability to maintain confidentiality of resident information (HIPAA compliance)
  • Proficiency in Microsoft Office (Word, Excel, Outlook)
  • Ability to sit for extended periods while performing clerical tasks
  • Occasional walking throughout the facility to coordinate with staff and residents
  • Ability to lift up to 25 lbs (files, supplies, equipment)
  • Manual dexterity for typing, filing, and handling documents
