What you bring to SBHG: (Must meet one of the following)
- High School Diploma (or equivalent): Requires a minimum of six (6) years of direct service experience in the mental health field.
- Associate degree: Requires a minimum of four (4) years of direct service experience in the mental health field.
- Bachelor’s Degree: Requires a minimum of two (2) years of experience working in the mental health field.
License or Certification:
- Valid California Driver's License (must meet agency criteria)
How you will make a difference (job overview):
This position has primary responsibility for providing oversight and care management to the clients referred to Star View programs. The Case Manager will provide services that may include case management/linkage, assessments, screenings, treatment planning/monitoring, and rehabilitation services to the clients and their families.
Division/Program Overview:
- Population Served: Children, Adolescents, and Transition-Age Youth (TAY)
- Facility & Setting: Field-based / Community settings (schools, homes, and local community facilities)
- Clinical Services: Comprehensive mental health care, psychiatric evaluation, Evidence-Based Practices (EBP), individual/family therapy, and multidisciplinary care delivered by psychiatrists, psychologists, social workers, and clinicians
- Length of Stay: Outpatient/community-based timeline (varies according to individual treatment plan and progress)
- Skills & Transition: Interagency collaboration and transition support with school administrators, county social workers, probation officers, and case managers. Case management, parent partner support, life skills development, and wraparound community support services
