Responsibilities

Patient Experience 

  • Coordinates the development, implementation, and ongoing evaluation of St. Francis Services and Telemedicine Services within St. Francis Regional Medical Center and in all partnering facilities.
  • Conducts patient rounds while utilizing clinical expertise to aggregate, assess and interpret information. Reviews Patient encounters, identifying items for performance/process improvement.
  • Coordinates stroke performance improvement, patient safety forums, and committees.
  • Manages new stroke initiatives by identifying and overcoming barriers to clinical care. Manages all aspects of the stroke program using planning, execution and monitoring and controlling processes.
  • Responsible for issue identification, issue validation, issue analyses and initiates follow-up to assure loop closure to improve clinical outcomes and patient care.

Financial Success

  • Identify areas of improvement; using this data to develop educational programs, action plans to address quality issues identified and to ensure follow through of developed plans.
  • Conduct analysis of program functions and needs and determine best evidence-based practice that evaluate the Stroke Program, work with program directors and managers to implement practice protocols.

Operational Excellence

  • Serves as a leader of the stroke workgroups/committees and is an active driver in strategic planning for the Stroke Program. Communicates and coordinates with departments and services on a daily basis to provide an efficient and quality service.
  • Oversees the preparation for Joint Commission certification surveys.
  • Serves as a resource for staff regarding quality and process improvement recommendations. Serves as a resource for staff regarding quality and process improvement recommendations.

Quality and Patient Safety

  • Identify areas for improvement and education, using stroke PI data to develop education programs to address quality issues identified in the stroke program.
  • Collaborates with multi-disciplinary care team to promote optimal outcomes for stroke patients.

Team Member Experience:

  • Counsels and trains staff members to increase awareness of adverse trends and participation in interdisciplinary process improvement.
  • Instructs others on pertinent standards and guidelines; ensures these standards and guidelines are adhered to.
  • Serves as a resource/mentor to bedside staff within the stroke service line.

Qualifications

Work Experience:

  1. Current RN license with a minimum of 3 years of experience 

License & Cert :

  1. Holds a current Louisiana license without restrictions as a health care professional.
