Community Health Centers of America is a premier multi-specialty group focused on improving the health of the community through excellence in clinical care and at contracted care facilities. We are looking for an LVN to work full-time with us in our Fair Oaks office as a clinical quality resource supporting CHCA's Skilled Nursing Facility (SNF) and Board & Care (B&C) programs primarily in the greater Sacramento area.

The Licensed Vocational Nurse (LVN) focuses on improving documentation quality, regulatory compliance, communication between CHCA and partner facilities, and operational efficiency. The LVN acts as a liaison between CHCA providers, facility staff, and internal departments to identify and resolve clinical and operational issues, ensure accurate and complete documentation, support quality improvement initiatives, and facilitate coordination of care. While the position may assist with Annual Wellness Visits (AWVs), Transitional Care Management (TCM) visits, and other preventive care initiatives, its primary focus is ensuring high-quality clinical operations and documentation across CHCA's post-acute programs.

Essential Duties and Responsibilities

  

Quality Assurance & Documentation Review
  • Perform routine chart audits to ensure clinical documentation is complete, accurate, and compliant with      organizational, payer, and regulatory requirements.
  • Review provider documentation for completeness, signatures, coding support, care plans, medication      reconciliation, and required clinical elements.
  • Identify documentation deficiencies and coordinate timely completion with providers and clinical      staff.
  • Assist with preparation for internal quality reviews, payer audits, and regulatory surveys.
  • Monitor documentation trends and recommend process improvements to enhance quality and efficiency.

Clinical Operations & Facility Support

  • Serve as a primary liaison between CHCA and partner Skilled Nursing Facilities and Board & Care      homes.
  • Build collaborative relationships with facility administrators, Directors of Nursing, social services, and      interdisciplinary teams.
  • Assist in identifying, investigating, and resolving operational issues affecting patient care, provider workflow, documentation, scheduling, or communication.
  • Escalate concerns appropriately and assist with implementation of corrective action plans.
  • Support onboarding of new facilities, providers, and clinical initiatives as assigned.
         - Perform delegated clinical services within the LVN scope of practice as assigned, which may include reading tuberculosis (TB) skin tests, performing bone density scans, obtaining basic clinical measurements, or assisting with other preventive health and quality initiatives.

Quality Improvement & Care Coordination

  • Participate in quality improvement initiatives designed to improve patient outcomes, reduce      avoidable hospital readmissions, and strengthen care coordination.
  • Assist with tracking quality metrics and identifying opportunities for process improvement.
  • Collaborate with providers and administrative teams to ensure timely follow-up on laboratory results,      referrals, diagnostic testing, and other outstanding clinical items.
  • Support care coordination activities related to transitions of care, Annual Wellness Visits (AWVs),      Transitional Care Management (TCM), and other population health initiatives when needed.

Communication & Provider Support

  • Facilitate communication between CHCA providers, facility staff, specialists, and internal departments.
  • Assist providers in obtaining information needed for patient care and documentation.
  • Coordinate follow-up regarding incomplete records, orders, referrals, or clinical concerns.
  • Help ensure continuity of care through effective communication and timely problem resolution.

Reporting & Compliance

  • Maintain quality tracking logs and assist with preparation of operational and quality reports.
  • Monitor completion of assigned documentation and quality initiatives.
  • Support compliance with FQHC, Medicare, Medi-Cal, and organizational policies and procedures.
  • Participate in quality meetings and contribute to ongoing performance improvement efforts.

Requirements

 Qualifications: 

  • Graduate of an accredited Licensed Vocational Nursing program.
  • Current, unrestricted California LVN license.
  • Minimum one year of experience in Skilled Nursing Facilities required.
  • Experience with clinical documentation review, quality improvement, utilization review, case management, or care coordination preferred.
  • Familiarity with electronic health record systems (PCC, eClinicalWorks, Epic, or similar).
  • Strong understanding of SNF regulations, interdisciplinary care, and clinical documentation standards.
  • Excellent organizational, communication, and problem-solving skills.
  • Ability to work independently while collaborating effectively with multidisciplinary teams.
  • Proficiency with Microsoft Office, including Excel and reporting tools.
  • Commitment to CHCA’s mission of providing compassionate, high-quality care to underserved populations.

Skills Required
