JOB TITLE: LVN CHARGE NURSE

REPORTS TO: DIRECTOR OF NURSING 

JOB SUMMARY: Responsible for providing nursing care to residents as prescribed by the physician and in accordance with standards of nursing practices, regulations, and directed by the Director of Nursing. The Charge Nurse will lead or direct licensed and non-professional staff in the delivery of direct Resident care and support functions. 

ESSENTIAL FUNCTIONS:

  • Provide the best possible nursing care by planning, organizing, and directing the nursing functions of patients in the unit
  • Make nursing assignments appropriate to the skill level of employees
  • Maintain acceptable standards of patient care
  • Identify problems and guides personnel to their solution
  • Create a working climate that provides growth and job satisfaction of personnel
  • Accurately and promptly implement physicians’ orders
  • Support and enforce infection-control policies and procedures
  • Use system to convey/receive resident information
  • Maintain a professional approach with confidentiality
  • Cooperate and maintain good rapport with nursing staff, medical staff, other departments, and visitors
  • Obtain and document nursing history from the patient and/or significant other that is comprehensive of present condition and prior problems
  • Perform basic assessment of the patient, indicating adequate knowledge of anatomy, physiology, and pathology
  • Record vital signs, note changes, and pursue more-specific investigation as needed
  • Assess for and notify physician and other appropriate parties of changes in condition
  • Use assessment information to develop a care plan before the end of duty time that communicates enough information for incoming personnel to adequately care for the patient 
  • Use care plans as a basis for providing safe and therapeutic care to patients
  • Consistently follow established standards, policies, and procedures in providing nursing care
  • Accept accountability for clinical care of assigned patients, including supervision of nonprofessional personnel
  • Communicate with the patient and his or her family and other members of the health care team in carrying out the goals of the care plan
  • Perform competently those technical skills considered basic to the type of care given in that unit
  • Respond appropriately in urgent and/or emergency situations
  • Support standards of nursing care through adherence to existing policies and procedures
  • Seek guidance when encountering nursing procedures and responsibilities that have not been mastered 
  • Promote positive public relations with patients, residents, family members, and guests 
  • Use tactful, appropriate communications in sensitive and emotional situations
  • Use appropriate workplace behavior and adhere to dress code at all times
  • Understand and adhere to the guidelines of “Residents Rights” and assure resident safety

Requirements

QUALIFICATIONS:

Required

  • Current LVN license
  • CPR certified required; 30 days will be given to complete a course and get certified
  • Ability to understand, remember and carry out verbal or written instructions in English
  • Ability to complete assignments in a timely manner
  • Ability to be flexible and positive with intermittent interruptions

Preferred:

  • 2 years nursing experience in long term care/skilled nursing

PHYSICAL REQUIREMENTS: The ability to safely perform movements such as pushing, pulling, lifting, bending, kneeling, and reaching up to 50 pounds with or without reasonable accommodations 

WORK SCHEDULE: As assigned, including some weekends, evenings and holidays 

WORK ENVIRONMENT: Clinical
