As a Licensed Practical Nurse (LPN), you will provide skilled nursing care to residents while supporting the interdisciplinary team to ensure high-quality, compassionate care.
Key Responsibilities- Deliver direct patient care in accordance with individualized care plans
- Administer medications and treatments as prescribed
- Monitor resident conditions and report changes to the care team
- Support and supervise CNAs to ensure quality care delivery
- Maintain accurate and timely clinical documentation
- Promote a safe, clean, and respectful environment
- Collaborate with interdisciplinary team members
- Active, unencumbered LPN license in South Carolina (required)
- Current CPR certification required
- Must maintain all continuing education and remain in good standing with the State Board of Nursing
- Experience with PointClickCare (PCC) preferred
- Strong clinical, communication, and teamwork skills
- Compassion for an underserved population

Skills Required
