Position Summary

Volare Health is seeking an experienced and detail-oriented Licensed Practical Nurse (LPN) to join our team as an MDS Nurse.

The MDS Nurse works closely with the interdisciplinary team to assess resident needs, coordinate the assessment process, and ensure accurate and timely completion of required assessments in accordance with state and federal regulations.

Key Responsibilities
  • Participate in the pre-admission process and obtain essential clinical information from referral sources to support accurate MDS planning.
  • Collaborate with the interdisciplinary team to review resident care and determine the most appropriate Assessment Reference Date (ARD).
  • Ensure accurate, timely, and compliant completion of resident assessments in accordance with state and federal regulations.
  • Complete accurate MDS coding using information gathered through resident and family interviews, staff communication, direct observation, and medical record review.
  • Maintain accurate MDS schedules and monitor assessment timelines.
  • Collaborate with nursing, therapy, social services, and other members of the interdisciplinary team to support comprehensive resident care.
  • Perform additional duties and responsibilities as assigned.
Qualifications
  • Current, active Licensed Practical Nurse (LPN) license in the state of practice.
  • Previous MDS experience preferred.
  • Knowledge of the Resident Assessment Instrument (RAI) process and case management.
  • Strong interpersonal and communication skills.
  • Excellent organizational skills and attention to detail.
  • Ability to work collaboratively with an interdisciplinary team.
  • Strong understanding of regulatory requirements and resident-centered care.
