Big Bend Hospice is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Home Care team. What You'll DoWorking under the direction of a Registered Nurse, you'll provide compassionate hospice care to patients in their homes while supporting families through every stage of the hospice journey. Responsibilities include:
- Deliver skilled, patient-centered hospice nursing care in the home setting
- Monitor symptoms and assist with medication management
- Document patient visits accurately and timely in the electronic medical record
- Communicate patient needs and changes in condition with the interdisciplinary team
- Educate patients and caregivers on medications, equipment, and safety
- Assist with care planning and support the RN Case Manager
- Help ensure patients remain comfortable, safe, and supported in their preferred environment
These duties are consistent with the position's responsibilities for providing hospice care, monitoring symptoms, educating families, documenting visits, collaborating with the interdisciplinary team, and supporting RN case managers.
QualificationsQualificationsRequired
- Current Florida Licensed Practical Nurse (LPN) License
- Minimum of one year of medical-surgical inpatient nursing experience - HOSPICE or critical care experience preferred.
- Valid Florida driver's license
- Reliable transportation and current auto insurance
- Strong communication and organizational skills
- Ability to work independently while collaborating with the interdisciplinary team
Preferred
- Hospice or Home Health experience
- Experience providing compassionate end-of-life care
