Under the direction and supervision of the Home Health Care RN Case Manager ,assisting with care coordination activities and completing assigned visit frequencies according to the Patient’s Plan of Care. Duties at times may include various office duties such as chart review, and back up for the Intake Coordinator. Other duties may include coverage for Home Health Aides when needed during periods of increased census or staffing needs in order to meet regulatory compliance.
This position is responsible for providing skilled nursing intervention within the Licensed Practical Nurse scope of practice.
Skills Required
- Licensed Practical Nurse (LPN) license
- Provide skilled nursing interventions within the LPN scope of practice
- Assist with care coordination and complete assigned visit frequencies per plan of care
- Perform office duties including chart review and act as backup for Intake Coordinator
- Ability to cover Home Health Aide duties when needed
- Available for full-time day-shift work
