Job Description:

Licensed Practical Nurse (LPN) Clinical Coordinator

Schedule: Monday–Friday | 8:00 AM – 5:00 PM | In-Office Position

We are seeking a dedicated Licensed Practical Nurse (LPN) Clinical Coordinator to join our growing team. In this role, you'll play a vital part in coordinating patient care, supporting clinical operations, and ensuring patients receive the right care at the right time—all while making a meaningful impact in the lives of those we serve.

What You'll Bring
  • Current LPN/LVN license

  • Minimum of 2 years of LPN/LVNexperience preferred

  • Home health experience preferred

  • Knowledge of Medicare and Medicaid regulations, benefits, and home health practices preferred

  • Strong organizational and time-management skills

  • Excellent communication and interpersonal abilities

  • Positive, patient-centered approach to care

  • Commitment to delivering exceptional clinical outcomes and service

Skills Required
