LOCATION: Dublin, GA to Eastman, GA / Float between assigned locations based on staffing needs.
The Community Service Board of Middle Georgia is seeking a qualified individual to serve as our next LPN. This person will be responsible for maintaining complete, accurate healthcare records and reports. Documenting vital signs and psychiatric/medical information. Working with the medical staff to ensure that the health and safety of our clients are being best taken care of.
Responsibilities of the LPN:
- Working with physicians, PA's, NP's via zoom and in clinic.
- Maintaining complete, accurate healthcare records and reports.
- Medication administration, including injections.
- Monitoring patients for side effects or reactions. Phlebotomy.
- Documenting vital signs and psychiatric/medical information.
- Monitoring, recording, and reporting symptoms or change in patients conditions and responses.
- Effectively communicating with patients and their families, providers, and others.
- Complete prior authorizations and patient assistance applications in a timely manner.
- Return phone calls and answer messages.
- Ability to multitask while providing a high standard of care to our patients and be able to work autonomously as a team.
- Maintain HIPAA regulations.

- High School Diploma/GED Equivalent
- Valid Georgia Driver’s License
- Effective verbal and written communication skills
- Strong interpersonal skills and the ability to work effectively with diverse communities
- Ability to work independently and in collaboration with others
- Experience with Microsoft 365 Office Products
- Ability to organize, prioritize and meet deadlines accordingly
- LPN must be licensed to practice in the state of Georgia,
- Must possess basic computer and typing skills,
- Ability to work in Adult Outpatient
