POSITION SUMMARY:
The Licensed Practical Nurse (LPN) at our Certified Community Behavioral Health Clinic (CCBHC) provides medical support and care coordination to individuals and families, integrating physical and behavioral health services. Under the direction and supervision of a registered nurse or licensed provider, the LPN delivers care within the scope of practice defined by the Kentucky Board of Nursing (KBN). This role emphasizes collaboration within an interdisciplinary team to ensure holistic, patient-centered care. Care can include but not limited to substance use disorder, mental health disorders or dual diagnosis.
DUTIES AND RESPONSIBILITIES:
• Coordinate care plans and communicate with internal and external healthcare providers, under supervision, to support comprehensive, integrated care.
• Conduct basic health screenings within LPN scope, including vital signs, symptom checklists, and medication reconciliation.
• Use active listening and empathy to help patients identify their goals and make positive decisions, wellness coaching, and resources to promote positive outcomes.
• Review screener tools to help identify patient needs and determine appropriate nursing interventions.
• Perform focused reviews of systems and monitor for changes in health status, reporting concerns to supervising RNs or providers.
• Accurately document patient encounters and services in the Electronic Health Record (EHR) in accordance with agency policies and KBN standards.
• Provide first aid, support medication adherence education, and monitor for side effects or safety concerns under supervision.
• Conduct post-discharge follow-ups for patients recently hospitalized or discharged from crisis stabilization units, ensuring continuity of care.
• Facilitate referrals and follow-up coordination, tracking progress and ensuring patients access appropriate services.
• Prepare and submit end-of-day summaries and activity reports to support communication and workflow.
• Run basic reports, assist with chart reviews, and identify recurring patterns to improve care quality and patient outcomes.
*Facilitate admissions to the OBOT (Outpatient Based Opiod Treatment) Program.
*Work with MCO’s to obtain authorizations for services and medications.
*Facilitate provider orders and work as a member of the inter-disciplinary team to support comprehensive care including supervision of medication administration, shot administration and review of medical logs.
*Make appropriate medical recommendation and referrals if needed.
*Complete all medication documentation in a timely and acceptable manner.
*Collection of lab specimens including but not limited to blood and urine.
*Provide nursing coverage in any CCBHC outpatient clinic, program and/or residential facility as needed.
*Be available for on-call telephone calls, questions, concerns, issues, or evaluations as they arise/as needed.
*Other duties as assigned by supervisor.
QualificationsQUALIFICATIONS:
• Knowledge of LPN scope as outlined in KRS Chapter 314 and KBN advisory opinions.
LICENSURE/CERTIFICATION:
• GED or High School Diploma and an Active, unencumbered Licensed Practical Nurse (LPN) license in the state of Kentucky.
• Current CPR certification.

Skills Required
