What You’ll Do:

  • Provide skilled nursing care to patients in their homes in accordance with the plan of care developed by the RN.
  • Perform clinical tasks including medication administration, wound care, and monitoring of patient conditions.
  • Observe, document, and report changes in patient status to the supervising RN and care team.
  • Educate patients and caregivers on disease management, medications, and preventive care to support recovery and independence.
  • Promote patient and family engagement in care to improve health outcomes and self-management.
  • Assess the home environment for safety and make recommendations to reduce risks and support recovery.
  • Collaborate with an interdisciplinary team to support patient goals and continuity of care.
  • Participate in health education and preventive care initiatives within the community.

What You’ll Bring:

  • Graduate of an accredited practical nursing program; successful completion of an approved pharmacology course.
  • Current LPN license in the State of Maine and CPR certification required.
  • Minimum of 2–3 years of recent nursing experience in acute care, rehabilitation, or home health setting.
  • Strong clinical assessment skills with the ability to work independently in a home setting.
  • Experience with wound care, medication management, and chronic disease education preferred.
  • Excellent communication and documentation skills, with the ability to collaborate effectively with an interdisciplinary care team.
  • Reliable transportation, valid Maine driver’s license, and proof of auto insurance required.
