JOB DESCRIPTION
Position: Lead ALF Case Manager
Pay Range: $30.00-$35.00 PER HR
Reporting To: ALF Manager
Work Type: Remote/Field
POSITION SUMMARY:
The Lead ALF Case Manager is responsible for ensuring the positive quality of life and well-being of selected ALWP and CCT clients in a specified region of RCFE’s. This will be done through performing a monthly face-to-face visit with each client, as well as a weekly follow up visit with each client, their social worker, or their RN to ensure they continue to flourish in the RCFE. The Lead ALF Case Manager will identify services needed for each client and work with the CCT and/or ALWP coordinators, the CCT Manager, and the ALW/ALF Manager to ensure services are provided. The Lead ALF Case Manager will build rapport with the staff members at each RCFE and will work to ensure Libertana maintains positive communication with them. Quality assurance reviews may need to be performed as needed. The Lead ALF Case Manager will also help to develop PR strategies, manage client databases, and assist the Director of Waiver Services in general program development.
QUALIFICATIONS:

Minimum of 2 years’ experience working in a related field.

Must have a bachelor’s degree in social work, psychology, counseling, rehabilitation, gerontology, sociology, or related field.

Possess current CPR certification or can receive upon hiring.

Knowledge of California Community Transitions and California Assisted Living Waiver Programs preferred.

Exercises exceptional judgement and understanding of clinical practices and documentation skills.

Superior organizational skills and ability to work independently.

Good written and verbal communication skills.

Application management and Public Relations skills

Knowledgeable in the use of computers and electronic information and charting systems.

Ability to build and maintain good relationships with Libertana office personnel and RCFE staff.

Licensed driver with automobile in good working order.
ESSENTIAL DUTIES AND RESPONSIBILITIES:

- Conducts assessments using ALWP Monthly Form, and any other tools required to determine level of care and services needed for clients.
- Responsible for the completion of relevant documents and assessments.
- Uses judgement and decision-making abilities to assess client needs.
- Notifies ALWP Coordinator, CCT Manager, ALW/ALF Manager, of services needed for each client and ensures these services are provided in a timely manner.
- Notifies SNF Administrator if client has been re-hospitalized.
- Conducts quality assurance audits/assessments as needed.
- Establishes and maintains positive relationships with RCFE staff and other vendors.
- Significant amount of driving may be required throughout the Los Angeles, San Diego, Riverside, Fresno and San Bernardino County.

PHYSICAL REQUIREMENTS:

Stand, sit, talk, hear, and use of hands and fingers to operate computer, telephone, and keyboard on a frequent basis up to 40% of the time.

Reach, stoop, kneel and bend up to 20% of the time

Moderate amount of walking up to 15% of the time.

Moderate amount of driving up to 25%of the time.

Close vision requirements due to computer work on a frequent basis

Light to moderate lifting may be required up to 25lbs on a frequent basis.
