About the Role 


Atlas seeks to address the risks and needs of individuals released on their own recognizance citywide who are at heightened risk of future victimization or justice system involvement. FF-CM is an intensive, family-focused, case management approach that is designed to motivate and link families to resources and services to address referral behaviors and family risk factors. FF-CM is based on the same principles of the evidence-based FFT therapeutic approach: relational-based, strength-based, and respect-based. A key difference, however, is that the FF-CM service is focused on creating linkages to existing community resources to facilitate change. To do so, interventionists first engage families into the change process and motivate them to connect to services. The second part of the process involves Supporting and Monitoring families as they participate in services. Finally, interventionists help the families to Generalize gains and new skills by locking in these changes and planning for the future. The goal of the implementation of the FF-CM model is to increase the engagement of young adults into therapeutic services, namely Functional Family Therapy. 



What You’ll Do

  • Provide virtual/home-based* Functional Family Case Management to adults and their families, adhering to model fidelity requirements. (*as post-pandemic working conditions allow). Each Interventionist will carry a caseload of 10-15 individuals/families who reside in NYC and are referred to the Atlas Program. 
  • Provide relationally focused support to individuals and families in-home and/or in the community, wherever it is most convenient and appropriate for the client. Including resources and accesses to resources, and multiple opportunities for linking clients to family therapy services, particularly FFT.
  • Actively participate as part of the FFCM team, including attending weekly team consultation, group and individual supervision meetings.
  • Maintain timely documentation of case progress notes as required by FFT (within 48 hours). 
  • Participate in all FFT training, supervision, and consultation activities.


Your Qualifications: 

  • Bachelor’s Degree from an accredited university.
  • At least two (2) years of experience in community work.
  • Experience working with adolescents and knowledge of the juvenile justice and criminal justice systems. 
  • Proficient knowledge of the New York City Department of Education system, New York City criminal legal system, and community-based resources available to families in New York City.

This is a full-time position. Days of work are Monday through Friday with travel in the New York City 5 borough vicinity. Weekend flexibility required.

 

Additional Requirements:

  • Authorized to work in the U.S.
  • Ability to work in-person in New York City (NY) for at least 3 days or more per week.
  • Ability to travel to other Rising Ground sites (if required)
  • Ability to travel within the community and do visits
  • Ability to work some weekends as needed
