Schedule: Full-time, 40 hours per week, Monday–Friday. Typical schedules include 8:00 AM–4:00 PM or 9:00 AM–5:00 PM.

Earn $21.01/hour or $23.01/hour with the selection of Enhanced Pay.

Position Details

The Intensive Case Manager provides high-quality, community-based mental health services to adults living in Cumberland and Perry Counties. This role focuses on reducing hospitalizations, promoting independence, strengthening support systems, and supporting vocational and educational goals.

Key Responsibilities

  • Travel throughout Cumberland and Perry Counties to meet clients in their homes and communities.
  • Deliver Intensive Case Management (ICM) services to adults with serious mental health needs.
  • Conduct assessments, develop service plans, and provide ongoing support through linking, monitoring, advocacy, and skill-building.
  • Support clients in accessing psychiatric care, community resources, and social supports.
  • Provide education and support to families and significant others.
  • Maintain accurate and timely documentation in compliance with Merakey Behavioral Health, County, and State regulations.
  • Participate in clinical supervision, staff meetings, and quality improvement initiatives.
  • On-call for caseload.

Preferred Qualifications

The ideal candidate will possess the following qualifications:

  • Bachelor’s degree in sociology, social work, psychology, or a related field; OR high school diploma/GED with 12 college credit hours completed in relevant coursework, plus two years’ experience in human services with one year in direct client contact; OR a registered nurse (RN) license.
  • Valid driver's license and reliable transportation required.
  • Experience working with adults in public or private human services is preferred.

  • Bachelor's degree in sociology, social work, psychology, or related field OR high school diploma/GED with 12 college credits plus two years' human services experience including one year direct client contact OR registered nurse (RN) license.
