General Purpose of the Job

Responsible as a team leader for the development and implementation of individualized programs and services for individuals served in Day Program. Service areas emphasizing community integration and in-house activities to enhance the person’s chosen goals affecting his/her capacity to live, work, socialize, and participate within the community.

 

Essential Duties and Responsibilities

  1. Directs services as a team leader to people served based upon their choice and person-centered planning in their Life Plans/CFA’s.
  2. Responsible for assessments, development, and writing of the Day Program plans and addendums as needed.  Also responsible for attendance records, transportation data, daily documentation, monthly summaries, and prepare for and attend annual and semi-annual reviews of Life Plans/CFA’s. Responsible for all mandated/required documentation in the individuals case records.  
  3. Timely documentation of Day Program plans, documentation needing yearly updates, DDP’s, evaluations, reports. Responsible for all mandated/required documentation in Individuals case Records.
  4. As a team leader, directing staff to coordinate and implement all daily activities including community integration, in-house activities, skills training, guest speakers, volunteer activities, adaptive technology, and physical assistance. May include substitute duties for absent staff.
  5. Serves as team leader directing and assisting, as needed, in providing services to people for their physical and personal hygiene needs. Follows diet and eating/feeding protocols, level of supervision, behavior plans, toileting, repositioning, or ambulation as prescribed by a physician or PT consult; including any additional person specific plans needed for participant.  Proven competency with lifting using a variety of mechanical lifts.
  6. Responsible for reporting information regarding people’s well-being to the designated supervisor re: advocacy, health, and emotional issues. Communicate any or all issues to the appropriate professional.
  7. Adherence to all rules, regulations, policies, and procedures of Federal, State, and Agency Programs.
  8. Provides transportation and outings into the community; including but not limited to daily driving individuals served in Agency vehicles. This also includes the proper tie downs of wheelchairs and operation of lifts for loading and unloading.
  9. Responsible for maintaining van logbooks, program equipment, Day Program site, and Agency cleanliness and equipment. May include washing windows, removing snow and steam cleaning interiors.
  10. Supports and enhances the People served as the major decision makers affecting their services. Support Self-Advocacy and advisory committees.
  11. Participating in curriculum development activities\committee
  12. Other related duties as assigned by supervisor.

 

 

Qualifications

Education and/or Experience

High school diploma or general education degree (GED); two years of experience in related field working directly with persons with Developmental Disabilities preferred

Certificates, Licenses, Registrations

Valid New York State Driver's License

Successfully complete and maintain certification as assigned in the following areas: First Aid, AED, CPR, and AMAP.

 

 

 

 

Skills Required

  • High school diploma or GED
  • Two years experience working directly with persons with developmental disabilities
  • Valid New York State Driver's License
  • Certification and maintenance of First Aid, AED, CPR, and AMAP (must complete as assigned)
  • Proven competency with lifting using a variety of mechanical lifts
  • Ability to perform timely documentation, maintain case records, and prepare for plan reviews
