ABOUT THE ROLE:

The Wellness Nurse is responsible for providing nursing triage, health education, and risk reduction strategies to maximize the resident’s health and quality of life as defined by the 8 Dimensions of Wellness.

Here are a few of the daily responsibilities of a Wellness Nurse:

  • Assists residents in achieving their health and wellness goals, within the framework of the 8 Dimensions of Wellness, personal choice, and mind-body-spirit.
  • Provides support strategies and referrals for services to improve resident outcomes for chronic disease management and acute illness or injury. Acts as a resource for options and choices related to supportive strategies to improve health, function, and lifestyle. Develops a referral network of service providers in and outside the community in tandem with the Director of Resident Services.
  • Participates in the collaborative care weekly meeting. Communicates and collaborates with the multi-disciplinary team to strategize for optimal resident outcomes, campus flow, and occupancy.
  • Oversees the post-acute discharge planning and follow-up support process. Collaborates with health center and hospital staff to coordinate admissions, transfers, and discharges.
  • When on duty, attends to medical emergency calls and partners with residents to prevent further incidents through risk reduction strategies. Completes incident reports as per community protocol.
  • Provides annual updates for medical demographic information and advance directives.
  • Organizes and facilitates vaccination clinics.
  • Encourages utilization and supports provision of service with on-site healthcare practitioners.
  • Partners with Community Life Director and Resident Services Director to plan, implement and evaluate Wellness Programming, health screenings, and health education opportunities.
  • Pro-actively builds relationships with residents and seeks to establish trust, respect, and connection.
  • Communicates effectively with families and staff. Fosters and drives measures to improve community culture.
  • Maintains records appropriate for independent living and assures confidentiality of all resident information contained therein.
  • Attends in-service training and educational sessions as assigned.
  • Strives to achieve high levels of resident satisfaction. Uses sound judgement and strong organizational skills.
  • Other duties as assigned by the Resident Services Director.

Here are a few of the qualifications we need you to have:

  • Active LPN license for applicable state.
  • Three years of experience in a related organization preferred.
  • Has excellent observation and communication skills- both written and verbal.
  • Is self-directed with the ability to work with little supervision.
  • Is flexible and cooperative in fulfilling obligations.
  • Ability to problem-solve, make ethical recommendations, and define appropriate boundaries.
  • Complies with accepted professional standards and practice.
  • Must be comfortable with using computers and have basic skills with computer platforms such as Word and Excel.

Full Time       No Weekends or Holidays

Salary: From $26.00 per hour
