Position Summary: The Incident Investigator works closely with Leadership to conduct internal investigations based on any report of a client incident/complaint received from the NYS Justice Center, the Office of Mental Health, or incidents/complaints that are non-Justice Center and reported by Management, Clients or Staff.

  

Essential Duties and Responsibilities:

  • Conduct interviews with clients and employees after a notification from the NYS Justice Center, the Office of Mental Health or client/staff has been received. 
  • If Justice Center review is warranted (allegation of abuse or neglect or death of client) and a letter of determination is received with recommendations, review with appropriate leadership team members and IRC for closure of incident.
  • Gather all necessary evidence to establish if the charge given from the NYS Justice Center, Office of Mental Health or client/staff is Substantiated or Unsubstantiated.
  • Analyze all evidence and work with Leadership, Human Resources Director and/or the CEO for any necessary follow-up.
  • Manage NIMRS (New York State Incident Management and Reporting System) so that incidents are imported within 24 hours
  • Investigate, review at Incident Review Committee, implement corrective action plan, and close incident within the 45-day timeframe
  • Evaluate all aspects of the investigation to make fact-based determinations. 
  • Identify systemic issues and contributing factors to incidents that have occurred. 
  • Write detailed investigation to present to the appropriate Leadership and/or Human Resources Director.
  • Write clear concise Letters of Determination for all appropriate Leadership and/or Human Resources Director for each investigation. 
  • Set up meetings with Senior Leaders and/or Human Resources Director to present facts, examples of findings and recommendations.
  • Receive and record all Internal (non-reportable) Incidents.
  • Prepare monthly Incident Report Summary for the monthly Incident Review Committee meeting.
  • Present results of investigations at the monthly Incident Review Committee meeting.
  • Provide individual and group training on Incidents and Incident Reporting; other topics as required.
  • Manage a large case load of investigations. 
  • Other Duties as assigned.

Requirements

Physical Requirements:

  • Ability to occasionally lift up to 20 lbs.
  • Lengthy periods of sitting/standing at a desk and working on a computer.

Minimum Qualifications:

  • Bachelor’s Degree or Associates Degree and three years’ experience in investigations  
  • Field investigative experience - Investigative experience is defined as independent investigative experience and includes the following activities: interviewing witnesses; collecting and analyzing data and evidence; writing and summarizing case narratives. 
  • Must have a clean, valid NYS Driver License. 
  • Proficient with Microsoft Word and Excel.

Preferred Qualifications: 

  • Certified Investigator through OMH or the Justice Center, must provide proof of certification. 
  • Experience in the handling NYS Justice Center and non-NYS Justice Center investigations that deal strictly with client safety and reports.
  • Experience with OMH (NYS Office of Mental Health), OCFS (NYS Office of Children and Family Services) or any other oversight agency of NYS.
  • Knowledge and experience with HIPAA and other client record related legal requirements is also preferred.
