POSITION SUMMARY:
Case management positions consist of advocating and coordinating services for individuals with intellectual/developmental disabilities.

DUTIES AND RESPONSIBILITIES:
Develop person-centered service plans and conduct assessments as required by the program.
Conduct monthly face-to-face visits with participants.
Assist clients with accessing community resources.
Monitor participant progress with waiver services, including health, safety, and welfare in monthly contact notes.
Assist with building a network of supports.
Assure participant choice and access to community resources.
Return phone calls and emails promptly.
Complete and submit documentation by required deadlines.
Perform other related work as required and assigned.

 

Qualifications

PDS Qualification Option:
Bachelor’s degree in Social Work, Human Services, or a related field preferred.
Relevant fields may include Social Work, Psychology, Rehabilitation, Nursing, Counseling, Education, Gerontology, Human Services, or Sociology.
Relevant experience may include case management or direct work with individuals with intellectual/developmental disabilities, mental health conditions, or other vulnerable populations, including roles such as CNA, CMA, home health aide, personal care assistant, or similar. Unpaid experience such as internships, volunteering, and caring for a family member may also be considered.

Additional Requirements:
Valid driver’s license and auto insurance.
Strong organizational skills.
Ability to multi-task.
Excellent communication skills.
Self-motivated.
Ability to conduct and facilitate meetings.
Ability to collaborate with other team members.
Ability to advocate for the needs of participants.

JOB REQUIREMENTS / EXPECTATIONS:
This job consists of sitting to complete necessary paperwork on a computer.
Must be able to operate a motor vehicle and have reliable transportation with valid insurance.
Must provide verbal and written communication with team members.
Job setting is at the corporate office and/or home office setting.
Documentation is time sensitive and must follow waiver regulations.
Work schedule is Monday-Friday 8:00 AM-5:00 PM. Schedules can vary depending on participant needs and preferences for visits. Flexible working hours.
