We are hiring a Case Manager (Bilingual English & Vietnamese preferred) to join our team. In this position, you will assess Oregonians applying for, or currently receiving, long-term care services to develop, coordinate, and arrange a variety of services to allow older adults and people with disabilities to get the continuity of care they need. Summary of Duties

As an Case Manager, you will:

  • Contact Oregonians or collateral contacts monthly to set up and monitor service plan and safety needs. This could include in person, the phone, or electronic contact.

  • Work closely and collaboratively with the Oregonian, other agencies, medical and care providers, and community partners to provide benefits, services, and information.

  • Work with Oregonians to create person-centered risk mitigation plans and provide risk mitigation as needed.

  • Interview Oregonians to determine initial and ongoing eligibility for healthcare, social services, and protective service needs. This can include collateral interviews both in person and on the phone. Develop detailed, individualized, and person-centered case planning for Oregonians.

  • Inform Oregonians of rights, responsibilities, service options, and methods of recourse.

  • Utilize all applicable forms, computer systems, tools, and any other available resources.

Minimum Qualifications

A valid driver’s license and acceptable driving record are required for this position.

A bachelor's degree in a Behavioral Science, Social Science, or a closely related field

OR

A bachelor's degree in any field AND one year of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing)

OR

An associate degree in a Behavioral Science, Social Science or a closely related field AND two years of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing)

OR

Three years of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing).

Essential Attributes

We are looking for candidates with:

  • Experience coordinating services and support for individuals by assessing needs, developing service plans, monitoring progress, and adjusting services to promote positive outcomes 

  • Experience in a fast-paced environment delivering customer service to end users

  • Experience interpreting and implementing policy and procedures 

  • Experience preparing and maintaining written narrative documentation of actions taken 

  • Experience interviewing to obtain information to evaluate the next steps in the intake process

  • Working Conditions

    • Hybrid work is not an option during trial service. Hybrid work condition may be an option after trail service is completed and may be discussed with the supervisor.

    • Your work schedule may be flexible with option to work between 6am-6pm.

    • Fast paced environment with constantly evolving policies and procedures.

    • May drive a variety of different types of vehicles to remote places in a variety of terrain and possibly inclement weather.

    • Repetitive motions, prolonged sitting, and daily computer and technology use. 

    • May come in contact with animals, some which of may be dangerous.  

    • May be exposed to communicable diseases, such as the flu virus, Covid-19, and MRSA.

    • You will interact with a wide range of people including some who have experienced trauma and may exhibit challenges in controlling their emotions.

    • You may encounter stressful situations that require quick decision-making to ensure safety of yourself and others.

    • The Oregon Department of Human Services does not offer visa sponsorship.
