Vitality Hospice is seeking a motivated, relationship-focused Hospital Liaison to join our growing team. In this role, you will serve as a trusted resource to hospitals, healthcare providers, patients, and families, helping ensure timely access to compassionate hospice care when appropriate.

Responsibilities- Develop and maintain strong relationships with hospital case managers, discharge planners, physicians, social workers, nurses, and other healthcare professionals.
- Educate referral sources on hospice services, eligibility criteria, and the benefits of hospice care.
- Identify appropriate hospice candidates and facilitate timely referrals in collaboration with hospital staff and the clinical team.
- Meet with patients and families to explain hospice services, answer questions, and provide support during care transitions.
- Coordinate referrals and admissions with clinical team members to ensure a seamless transition to hospice care.
- Maintain regular contact with referral partners and provide exceptional customer service and follow-up.
- Participate in community outreach, networking events, and healthcare meetings to strengthen professional relationships and increase awareness of hospice services.
- Maintain accurate referral, account, and activity documentation within company systems.
- Support organizational growth initiatives through relationship development, education, and excellent service to referral partners.
- Conduct all outreach and referral activities in compliance with Medicare regulations, HIPAA requirements, and company policies.
- Healthcare marketing, hospice, home health, hospital discharge planning, case management, social work, nursing, or related healthcare experience preferred.
- Existing relationships within hospitals and healthcare systems in the assigned territory are a plus.
- Strong communication, presentation, and relationship-building skills.
- Ability to work independently, prioritize responsibilities, and manage a flexible schedule.
- Knowledge of hospice services preferred.
- Valid driver's license and reliable transportation required.
- Bachelor's degree preferred but not required with relevant healthcare experience.

Schedule: Monday–Friday
Work Location: Field-based position serving Burlington, Camden, and Gloucester Counties, New Jersey. Regular travel to hospitals, healthcare facilities, physician practices, and referral partners throughout the assigned territory is required.
