Job Overview
The NC Wraparound Facilitator will engage high-need youth and their families in developing, implementing, and monitoring an intensive, individualized care plan. This includes helping the team develop a natural support system, working on the development of an integrated plan and establishing a strong relationship with the family. This partnership is important as the youth and family work to address challenges and make changes to their lives.
The High-Fidelity Wraparound Facilitator is a person who safeguards that the principles and steps of the High-Fidelity Wraparound (HFW) process are delivered with the highest possible fidelity to the model. The NC Wraparound Facilitator helps the family develop a positive view of the future through various activities.
Responsibilities and Duties
• Works with family to integrate information from multiple sources into the strengths needs and culture discovery.
• Assists the family to identify appropriate members of the Child & Family Team (CFT)
• Collects background information and plans from other agencies.
• Facilitates the CFT to develop an integrated mission to support the family.
• Convenes and facilitates the CTS meetings.
• Ensures the HFW plan results in the best fit between the family vision, team mission, strengths, needs and strategies, through a proactive and reactive planning process that id inclusive of a connected crisis plan.
• Works directly with the youth and family to implement elements of the HFW plan.
• Prepares, monitors, and modifies the HFW Plan in concert with the family and CFT.
• Identifies, actively assists the youth and family to obtain, and monitors the delivery of available services including medical, educational, social, therapeutic, or other services.
• Assembles child and family teams: assesses strengths and needs of the family unit, coordinate meetings, seeks community resources and complete all necessary documentation
• Develops with the family a transition plan when the youth has achieved goals of the HFW Plan
• Collaborates with the other service providers and state agencies (if involved) on the behalf of the youth and family (examples: special education, mental health, child welfare, criminal/ juvenile justice)
• Facilitates reviews of the HFW Plan to reflect the changing needs of the youth and family.
• Completes the required tools as scheduled to track progress.
• Monitors and documents the status of the youth and family’s progress and effectiveness of the strategies and interventions outlines in the Plan of Care.
• Attends a minimum of one hour of group supervision and one hour of consultation per week to monitor adherence of the HFW principles.
• Possess a current, valid driver’s license and automobile with proof of auto insurance.
• Will complete all appropriate Federal and State background checks.
• Will maintain strict confidentiality according to all Federal and State guidelines and requirements.

Education/Qualifications/Experience
Master’s Degree Applicants:
• Must possess degree at time of application.
• Degree must be in a related field of study (refer to list below)
• Must have one-year, post-graduation, full-time experience (2,000 hours) with designated population (refer to list below)
Bachelor’s Degree Applicants:
• Must possess degree at time of application.
• Degree must be in a related field of study (refer to list below)
• Must have two years, post-graduation, full-time experience (4,000 hours) with designated population (refer to list below)
Experience Accepted:
• Experience should include clinical case management or practice of therapeutic interventions.
• At least two years of experience providing services for children and families.
• Experience facilitating small groups, preferably with multiple system involvement.
• Experience with case management database administration preferred.
• Experience must be with at-risk youth, i.e., youth with one or more of the following behaviors: Truancy, Alcohol/Substance Abuse, Homicidal/Suicidal Ideations, Problem Sexual Behavior, Verbal/Physical Aggression, Runaway, Antisocial Behaviors.
Training Requirements
Must complete CTS Health initial orientation training
HIPPA, Clients Rights, Confidentiality, CPI, First Aid, CPR, Blood Borne Pathogens, Workplace Violence, Case Management, Cultural Competency, Customer Service/Professionalism

Skills Required
- Master's degree in a related field (must possess at time of application)

- Two years post-graduation full-time experience (4,000 hours) with designated population for Bachelor's applicants
- At least two years experience providing services for children and families
- Experience including clinical case management or practice of therapeutic interventions
- Experience must be with at-risk youth (truancy, substance abuse, suicidal/homicidal ideation, problem sexual behavior, aggression, runaway, antisocial behaviors)
- Experience facilitating small groups, preferably with multiple system involvement
- Experience with case management database administration
