HHS Case Manager

Description

The Case Manager will be responsible for providing comprehensive housing-related services under the Bringing Families Home (BFH) and Home Safe grant initiatives. This role will support families, at-risk adults, and individuals experiencing homelessness by offering case management, housing assistance, and benefits advocacy. The Case Manager will work collaboratively with the ICWA Advocate, Victim Services Coordinator, and other relevant departments to ensure holistic client support. This is a grant-funded position.

Essential Duties and Key Responsibilities

• Conduct housing intake assessments and develop individualized service plans for clients.

• Provide case management services, including coordinating housing assistance, rental support, and benefits applications.

• Assist clients in locating suitable housing and work with landlords to secure placements.

• Ensure clients are document-ready for housing applications and other supportive services.

• Utilize SMART (Specific, Measurable, Achievable, Relevant, Time-bound) goals to guide clients toward self-sufficiency.

• Maintain confidentiality, particularly for ICWA cases and domestic violence survivors.

• Coordinate services with other tribal departments and external agencies to enhance client support.

• Maintain accurate case files, progress notes, and required program documentation.

• Attend regular team meetings and provide updates on program outcomes and client progress.

• Participate in ongoing professional development and training programs.

• Track client progress, maintain detailed records and ensure that services are provided according to the grant guidelines and budget. 

• Responsible for submitting grant reports by established deadlines. 

Skills & Abilities

• Experience in social service settings providing direct services to clients.

• Knowledge of housing programs, benefits applications, and social service resources.

• Knowledge of case management systems and case planning techniques.

• Proficiency in maintaining client records and program documentation.

• Proficient in Microsoft Office, data systems, and communication technology.

• Highly organized with strong analytical, time management, and communication skills.

• Ability to handle sensitive information with professionalism and discretion, maintaining confidentiality and professional ethics.

• Familiarity with trauma-informed care and culturally responsive practices.

• Excellent verbal and written communication skills.

• Strong problem-solving skills to help clients address housing and benefits-related challenges.

• Strong relationship building and collaboration skills to work with various departments and external agencies.

• Flexibility to adapt to changing client needs and program requirements.

• Ability to work independently with minimal direction.

Education and Experience

Required:

• Associate's degree from an accredited university with a major in a social work field, or behavioral science, social science, or other related field; or a High School Diploma and two years of on-the-job training and experience in case management, housing services, or social services.  

• Completion of a 40-hour Domestic Violence training course. (May be obtained within six months of hire.)

• Valid driver’s license and satisfactory motor vehicle report.

• Ability to successfully pass the required drug screen, background check and fingerprinting.  Employer observes federal standards for controlled substances, including cannabis.

Preferred: 

• Bachelor’s degree from an accredited university with a major in Social Work or related field of study.

• Experience working with Native American communities.

Physical Requirements

• Ability to sit, stand, walk, and perform light lifting (up to 15 lbs).

• Ability to use computers and communication devices.

• Ability to drive a motor vehicle for home visits and meetings.

• Requires specific vision capabilities, including close and distance vision.

Compensation: The hiring range for this position is from $25.99 to $ 32.28.  The compensation offered for this position will fall between this range based on the candidate’s level of applicable skills, experience, and education.
