ARISE/Exceptional Family Resources is looking for a Community Habilitation Specialist to join our team!
The Habilitation Specialist serves as the primary point of contact for new enrollments into Agency Community Habilitation, Respite, Family Support Services (FSS), and Take-a-Break programs. This position is responsible for coordinating intake, eligibility documentation, program transfers, and OPWDD system functions while supporting program continuity and compliance. The role combines administrative coordination with limited direct service delivery.
Essential Duties and Responsibilities- Serves as the primary contact for new enrollments in Agency Community Habilitation, Respite, FSS, and Take-a-Break programs
- Gathers, reviews, and organizes all enrollment and eligibility documentation
- Facilitates service-related transfers from habilitation programs to other programs
- Completes DDP and Drop/Add functions in OPWDD CHOICES
- Manages and maintains the intake list, including related communication with internal and external stakeholders
- Provides occasional caseload support as needed
- Provides new employee training on essential technology and related
- Completes a minimum of 20 hours of direct service per month
- Performs other duties as assigned to support agency operations
- Other duties as assigned
This position is required to complete 20 hours of direct service per month, in accordance with program and agency needs.
Qualifications- Associates degree or higher preferred. An equivalent combination of education and relevant experience will be considered.
- Experience with or knowledge of OPWDD regulations and requirements
- Experience with supporting people with developmental disabilities preferred
- Valid driver’s license and reliable transportation
- Must have device or other technology to download apps for use in community
