The Role
Embed with clinical and operations teams to identify high-impact problems and build production AI systems. Design and deploy LLM/RAG-driven tools, data pipelines, APIs, models, and user-facing applications using messy clinical and claims data. Iterate quickly based on real-world feedback and operational needs.
Summary Generated by Built In
Astrana is building a high-impact AI transformation team focused on solving the most important problems across the business. The role is straightforward: identify where the organization is losing time, outcomes, or value - and build systems that fix it.
This means working directly with the people running our markets, understanding how the work actually gets done, and building entirely new tools, automations, and applications from the ground up.
This position operates under a hybrid work model, with a combination of in-office and remote work each week. Office locations include Monterey Park, CA; Orange County, CA; and Las Vegas, NV.
What This Role Involves
You will embed directly within clinical, operations, and market teams and build systems that change how care is delivered and how the business performs.
In a given month, you might:
- Build an AI-driven tool to help care teams prioritize high-risk patients
- Automate a workflow that is currently manual and impacting revenue or cost
- Deploy an LLM-based system on top of messy clinical or claims data
- Create internal tools that providers and operators rely on every day
This role is about shipping real systems that work in the real world - not prototypes or isolated experiments.
Why this role - and why Astrana
Healthcare is nearly 20% of U.S. GDP, yet remains fragmented, inefficient, and misaligned.
Astrana is building a new category: a technology-driven healthcare platform that sits across payers, providers, and care delivery - integrating clinical, claims, and operational data and enabling decisions based on total patient outcomes, not isolated transactions. Astrana is building the operating layer for healthcare delivery.
This is not a concept. It is already working. Astrana today:
- $3.2B in revenue in 2025, growing 56% year-over-year
- 1.6M+ patients under management across a national platform
Why do this here vs somewhere else:
- You are early, despite meaningful scale
The platform is already large and growing quickly, but this capability is still being built - giving you outsized influence and career opportunity
- You can actually change the system, not just analyze it
Astrana operates under delegated risk, with real responsibility for cost and outcomes - so what you build can directly influence decisions and behavior.
- There is immediate, real-world deployment at scale
You are not building pilots or internal tools that may or may not be adopted. You are building into workflows used across a large and growing population.
- The incentives are aligned with the problem
Improving outcomes while reducing cost is the business model - making AI, automation, and coordination economically meaningful from day one.
- You have executive backing and speed
AI is a core priority with direct CEO sponsorship, enabling fast iteration from idea to production.
- You are surrounded by strong builders
Our team includes engineers and operators from leading tech companies (Google, Amazon, etc), top quantitative firms (Citadel, D.E. Shaw), and strong academic backgrounds.
What You'll Do
- Work directly with clinical, operations, and market teams to understand how work actually happens
- Identify high-impact problems affecting cost, quality, and patient outcomes
- Rapidly design and build tools, automations, and AI systems used in production
- Work with complex, messy datasets (clinical, claims, operational)
- Build across the stack: data pipelines, APIs, models, and user-facing applications
- Iterate quickly based on real-world feedback
- Help shape how technology is adopted across the organization
Who this is for
This role is for people who:
- Build things without being asked
- Get deeply curious and go down rabbit holes to understand problems
- Are comfortable operating with ambiguity and figuring things out from first principles
- Care about impact and usefulness more than perfection
- Want their work to be used immediately by real people
Strong signals
- You have built and shipped systems that real users depend on
- You have worked with real-world, messy data
- You have hands-on experience with LLMs, RAG systems, agents, or applied ML
- You can go from idea to working product quickly
- You have taught yourself tools or technologies
Qualifications
- Experience building and deploying production systems
- Strong technical fundamentals and problem-solving ability
- Preferred: B.S., M.S., PhD in Computer Science, Mathematics, Physics, or a related technical field
- 1-3 years of experience in software engineering, data engineering, ML, AI, or related fields (flexible for exceptional candidates)
We are open to exceptional new graduates and early-career engineers who demonstrate strong builder instincts and high potential.
