Join Benchmark as an Extended Services Case Coordinator to provide support to individuals in sustaining long-term employment following the closure of their vocational rehabilitation case.
Must be willing to travel the greater Fort Wayne area as well as Marion.

RESPONSIBILITIES:
- Maintain accurate client records as per program and regulatory requirements.
- Conduct regular meetings with individuals and employers on-site.
- Offer support in navigating workplace dynamics and fostering relationships.
- Facilitate re-referrals to vocational as necessary.
- Provide outcome data to designated personnel.
- Monitor and summarize monthly billing activities.
- Perform periodic file reviews for accuracy and timeliness.
- Coordinate all business between department and state funding bodies.
- Oversee client programming and services.
- Adhere to attendance requirements set by the program, Benchmark, and individual clients.
QUALIFICATIONS:
- Bachelor's degree in a related field and/or one (1) year experience preferred with people with disabilities
- Three (3) years experience of continued and verifiable employment within two years of submitting an application
- Valid Indiana Driver’s license and auto insurance
- Reliable transportation
