The Harris Center for Mental Health and IDD (“The HARRIS CENTER”) is looking for an Eligibility Coordinator, to join our team. The Eligibility Coordinator is responsible for managing, reviewing, and processing incoming referrals to ensure individuals meet program eligibility requirements. This role serves as the primary point of contact for referral sources, clients, and internal teams, ensuring accurate documentation, timely follow up, and smooth coordination of services. The Eligibility Coordinator verifies required information, gathers supporting documentation, enters referral data into agency systems, and communicates outcomes to stakeholders. This position ensures compliance with agency standards, state regulations, and program guidelines while supporting efficient service delivery and positive client engagement.

Your role in action:

  • Produces and proof correspondence, documents, and other related forms.
  • Maintains files according to organizational and/or departmental procedures.
  • Receive and process all incoming referrals from families, hospitals, physicians, community agencies, and internal teams.
  • Enter all referral data accurately into the Agency’s system within required timelines.
  • Maintain accurate, up to date referral records in accordance with state ECI regulations.
  • Secure all confidential information following HIPAA and program privacy standards.
  • Maintains professional behavior and provides excellent customer service to both staff and consumer.
  • Provides documentation of work activities and follows up on missing referral information.
  • Adapts to changing working requirements. Flexible to meet ongoing needs of the Agency.
  • Complies with Agency policies and procedures, accrediting/regulatory bodies and performance contract documentation requirements.
  • Performs other related duties assigned

Education:

·    High School Diploma or General Education diploma.

Experience:

  • Experience working with individuals with intellectual and/or developmental disabilities preferred.

Required Knowledge, Skills and Abilities:

  •  Basic understanding of common signs and symptoms of developmental delays and able to identify potential need for support.
  • Early Childhood Intervention/ Child Development knowledge preferred.
  • Strong communication skills, listening and interpersonal skills above average, written documentation is clear, concise, well- constructed and grammatically correct.
  • Able to work collaboratively with others in a team environment.
  • Effective time management and organizational skills.
  • Able to execute standard word processing, excel software and use of other computer databases as needed.
