JOB SUMMARY
Under the direction of the Manager of Case Management and the immediate supervision of the RN Case Manager and the clinical Social Workers, assists with completing concrete tasks necessary to facilitate post acute planning.
PERFORMANCE DUTIES
- Coordinates completion of and transmits information to post acute provider i.e. home care, nursing home, other discharge/transfer destinations and services as directed through the ECIN software system. May carry an assigned caseload of nursing home return patients.
- Contacts agencies regarding bed availability and accessibility and procedures for admission. Facilitates outpatient dialysis scheduling as directed and coordination of transportation needs.
- Develops strong working relationships with various community agencies in order to promote timely patient discharge and positive financial outcomes.
- Facilitates the ordering of Durable Medical Equipment as directed by the Case Manager.
- Investigates patient’s insurance coverage for outpatient and long term care needs.
- Documents per department guidelines in the electronic medical record.
- Maintains an electronic data base of community resources.
- Assists Appeals/Denials Manager as needed.
- Works on other projects as needed such as closing cases in Allscripts for reporting needs.
- Assists with the delivery of the Medicare Important Message to patients/families if needed
JOB QUALIFICATIONS
A.Education
- High school graduate
ØLicensure
B.Training and Experience
- Health Care related experience preferred
- Training in the use of EMR and Allscripts.
C.Job Knowledge
- Proficient computer skills that includes the internet, email, Word, and able to adapt to new programs quickly.

A.Physical
- Moderate physical demands due to the frequent need for rounding on the floors, carrying patient charts within a limited time-frame.
- Visual and aural acuity
- Good communication skills and pleasant telephone manner.
- Ability to triage customer enquiries.
- Able to prioritize and manage multiple requests by the case management team
