Perimeter Behavioral of Springfield is looking for a compassionate, organized, and mission-driven Discharge Planner to help children, teenagers, and families move confidently through treatment and into successful aftercare.

If you are passionate about service excellence and want to make a meaningful difference every day, we would love to meet you.✨ What You’ll Do- 🤝 Participate as an active member of the treatment team, including staff meetings, treatment planning meetings, and assigned committees.
- 📋 Coordinate with therapists, utilization review staff, doctors, and insurance companies to support documentation, discharge planning, and continuity of care.
- 💬 Help identify patient needs and work with the treatment team to ensure those needs are addressed.
- 👨👩👧 Partner with families and treatment teams to encourage involvement in each patient’s care.
- 🌍 Communicate with referral sources, mental health professionals, and community agencies to support aftercare planning and successful transitions.

- 🎓 Education: Bachelor’s degree in social work, psychology, or a related field.
- 🧠 Experience: One to Two years of experience in an inpatient/outpatient mental health setting, or comparable experience; strong knowledge of clinical treatment principles and social casework preferred.
- 📌 Training: Successful completion of CPR and Crisis Prevention Intervention Training within 30 days of employment.
- 🕘 Schedule: Monday–Friday, 8:00 a.m.–5:00 p.m.
