About Us Careview Transitional Care of Weston is a CMS 5-Star, DEFICIENCY FREE skilled nursing and rehabilitation facility in Weston, Wisconsin, known for high clinical standards, strong outcomes, and a busy, fast-paced, team-driven culture. Our reputation is built on getting patients home faster, stronger, and safer — and our Director of Business Development (DBD), also known as a Clinical Liaison/Marketer, is the face of that promise in the community.

Position Summary The DBD/Clinical Liaison is the bridge between our facility and the hospitals, physicians, and case managers who refer to us. This is a hybrid clinical and business development role — you'll be at the bedside performing pre-admission assessments one hour and meeting with discharge planners or marketing to referral sources the next. You will drive census, build and maintain strong referral relationships, and ensure every patient transitioning to our care is clinically appropriate and well-prepared for admission.*This is not a role for someone who wants a predictable day. It is ideal for a confident, organized clinician who thrives on autonomy, relationships, and results.
Overview of Position
- Location: Weston, WI (based) — Territory spans Marshfield to Wausau and surrounding areas as needed
- Discipline: Registered Nurse (RN), LPN (preferred), or Licensed Therapist (PT, OT, or SLP)
- Setting: CMS 5-Star, high-volume post-acute skilled nursing & rehabilitation facility
- Travel: Daily travel within territory; reliable transportation and valid driver's license required
Key Responsibilities
Census Development & Referral Management
- Drive facility census by securing and converting referrals from hospitals, physician groups, and community partners across the Marshfield–Wausau corridor.
- Build and maintain trusted relationships with hospital case managers, discharge planners, social workers, physicians, and other referral sources.
- Respond promptly to referrals — same-day turnaround is the standard.
- Track referrals, conversions, and territory activity; report metrics regularly to leadership.
Patient Assessment & Admission Coordination
- Conduct bedside pre-admission clinical assessments at referring hospitals to confirm clinical appropriateness and skilled need.
- Communicate clinical findings, equipment needs, and care requirements to the facility admissions and clinical teams for a seamless transition.
- Educate patients and families about our services, programs, and what to expect during their stay.
- Coordinate with the admissions team to verify insurance, authorization, and bed availability.
Marketing & Community Outreach
- Plan and execute marketing activity across the territory — in-services, lunch-and-learns, facility tours, hospital rounding, and community events.
- Represent the facility at networking events, professional associations, and healthcare functions.
- Identify and pursue new referral opportunities and partnerships.
- Collaborate with internal clinical, admissions, and leadership teams to align outreach with facility capabilities and capacity.
Qualifications
Required
- Active Wisconsin license as a Registered Nurse (RN), LPN, Physical Therapist (PT), Occupational Therapist (OT), or Speech-Language Pathologist (SLP).
- Strong clinical assessment skills with the ability to evaluate complex patients at the bedside.
- Excellent multitasking, prioritization, and time-management skills — this is a high-volume environment.
- Outstanding interpersonal and communication skills; comfortable presenting to and building rapport with physicians, case managers, patients, and families.
- Self-motivated and able to manage a territory independently.
- Valid driver's license, reliable transportation, and willingness to travel daily within the Marshfield–Wausau corridor (and beyond as needed).
- Proficient with computers, EMRs, and referral platforms.
Preferred
- Post-acute experience in inpatient rehabilitation, skilled nursing, long-term acute care, or hospital case management.
- Prior experience in a clinical liaison, marketing, business development, or admissions role.
- Established relationships with referral sources in central Wisconsin.
- Knowledge of Medicare, Medicaid, and managed care criteria for post-acute placement.
