Position Summary:
Reporting to the Program Director, Food Services, this position is responsible for providing comprehensive nutrition assessment, intervention, monitoring, and education to clients within a sub-acute neuro rehabilitation setting. The Clinical Dietitian works closely and collaboratively with clients, families, nursing, physicians, speech language pathology, occupational therapy, physiotherapy, and other members of the interdisciplinary team to support client centered goals, optimize nutritional status, and promote safe and successful rehabilitation outcomes.
 
Key responsibilities include completing comprehensive nutrition assessments using clinical judgement, standardized screening and assessment tools, and current evidence-based practices to identify and address nutrition related concerns. The Clinical Dietitian develops, implements, and evaluates individualized nutrition care plans that incorporate the client's medical and nutritional needs, functional status, rehabilitation goals, preferences, and input from the interdisciplinary team. The Clinical Dietitian provides nutrition care to clients with a range of neurological conditions and rehabilitation needs, including stroke, traumatic brain injury, and other neurological disorders.
 
The position requires an understanding of the nutritional considerations associated with neurological conditions, including dysphagia, cognitive and communication impairments, changes in functional ability, altered appetite and intake, diabetes management, bowel and bladder concerns, weight management, and increased or altered nutritional requirements related to recovery and rehabilitation. This position is responsible for initiating, managing, and monitoring enteral nutrition as required, including assessment of nutritional requirements, selection and adjustment of enteral feeding regimens, monitoring tolerance and adequacy, addressing nutrition related complications, and collaborating with the interdisciplinary team to support transitions between oral and enteral nutrition.
 
The Clinical Dietitian also provides education and support to clients, families, and care teams regarding nutrition interventions and enteral feeding plans to promote continuity of care and facilitate safe discharge planning. The Clinical Dietitian provides individualized and group-based nutrition education to support clients in developing the knowledge, skills, and confidence required to manage their nutritional needs during rehabilitation and following discharge. Education may be provided one to one or in group settings. The Clinical Dietitian adapts education to accommodate cognitive, communication, physical, and learning needs associated with neurological conditions and collaborates with the interdisciplinary team to reinforce education and support carryover. 
 
The Clinical Dietitian is also responsible for providing nutrition services and coverage to other programs within the site, including Chronic Complex Care, RCTP, and eQUAL, as operational needs arise. This requires the ability to adapt clinical practice to different client populations, care needs, and program environments.
 
The Clinical Dietitian is responsible for effectively prioritizing and organizing workload across multiple units and competing clinical demands, using clinical judgement to identify urgent and high-priority nutrition needs while ensuring timely follow up and continuity of care for all assigned clients.
 
In keeping with Carewest policies and practices, applicable professional standards, and current evidence-based practice, this position is responsible for documenting and maintaining all aspects of nutritional care in a timely, accurate, and comprehensive manner. The Clinical Dietitian participates in interdisciplinary rounds, admission conferences, discharge planning, education initiatives, and other team processes as required. The successful candidate demonstrates strong clinical assessment, critical thinking, communication, collaboration, and workload prioritization skills. Experience with neurological populations, dysphagia, enteral nutrition, and diabetes education is considered an asset.
 
Program summary: Neuro Rehabilitation Program – for adults who have had a neurological injury, such as a stroke, and require additional assessment, recuperation and rehabilitation before returning to the community.
 
Qualifications & Experience: 
  • Graduation from a recognized degree program in Foods and Nutrition;
  • Completion of recognized internship program Registration with the College of Dietitians of Alberta required;
  • Registration with Dietitians of Canada an asset
  • Experience as a Clinical Dietitian in a clinical and/or continuing care setting an asset
