The dream of Hopewell Center began in the early 1950's when a group of parents sought services for their children and founded Peter Pan Preschool. The group incorporated in 1966 and officially adopted the name Hopewell Center in May 1983.

As a private, not-for-profit agency, Hopewell has experienced rapid growth in services, personnel, and persons served. While originally focusing exclusively on persons with disabilities, Hopewell has expanded its services to families with infants and toddlers that are 'at-risk' due to developmental or socio-economic factors.

Hopewell now has an array of facility and community-based opportunities, including family services, children's services, employment services, and residential services. Today, nearly 200 caring professionals provide person-centered services to over 450 individuals a year.

Job Description

The Day Habilitation Supervisor trains and assists consumers in the performance of daily living activities within the facility and in the community, exercising some judgement to achieve goals and objectives of Individual Program Plans.  

Responsibilities

  • Implements Individual Program Plans (IPPs), assisting consumers to meet established goals and objectives; maintains documentation of performance.

  • Supervises and assists consumers in completing tasks and activities on time and following standard procedures regarding breaks; assists consumers to restroom, with eating lunch, and other activities, as needed.

  • Supervises consumers on community-based activities ensuring safety, education and access.

  • Implements behavior plans for consumers, rewarding consumers and/or correcting behavior, maintaining documentation of behavior, corrections provided and each consumer’s response.

  • Maintains a safe environment by reporting safety hazards and maintenance needs to superiors, instructs and supervises authorized consumers in the safe operation of equipment, and prepares accident reports in the event of consumer/staff injuries.

  • Completes a variety of consumer specific documentation of training and services provided, daily attendance and activity records, group logs, end of the month summaries and other required documentation through provided technology; prepares and submits required reports.

  • Provides support on an as needed basis to consumers in all activities while participating in training, including but not limited to tech support, self-care and socialization.

  • Periodically operates motor vehicle to transport consumers to various destinations, as needed or directed.

  • Evaluates consumer progress on their goals and advances them to appropriate goal objective. Completes and submits end of the month summaries.

  • Inventories training materials and keeps in good condition.

  • Oversees emergency evaluation training of consumers.

  • Performs related duties as assigned, assuring the best interest of both the agency and the consumers' welfare.

Qualifications

  • High school diploma or equivalence preferred;
  • Knowledge of proper methods to train developmentally disabled individuals;
  • Must obtain CPR and basic first aid certification.
  • Ability to effectively utilize basic technology platforms
  • Knowledge of Individual Program Plans (IPPs);
  • Ability to comprehend, interpret, develop and implement plans; Ability to maintain records of consumer activities and perform arithmetic calculations;
  • Ability to follow verbal and written instructions;
  • Ability to work independently and organize projects;
  • Ability to follow all personnel policies and rules of the organization and maintain confidentiality;
  • Ability to communicate with other agency personnel, consumers, families and guardians. 
  • Ability to satisfactorily complete all necessary in-service training, and ability to obtain CPR and basic first aid certification; Possession of a valid driver's license and demonstrated safe driving record
