Position Summary:

The Crisis Services Supervisor is responsible for providing day-to-day supervision of staff and program services for individuals who meet the criteria for the Crisis Services Program. Oversees the implementation of service plans, including medical care and behavior supports. Ensures that individuals' outcomes and objectives are being met and ensures supports provided meet agency guidelines, funder requirements and accreditation standards.

 

Essential Functions:

  • Works with individuals who have complex behavioral support needs. Supervises team members providing generalized positive behavioral interventions while Individual Support Plans (ISPs) and/or Behavior Support Plans (BSPs) are being developed/revised and supervises team members with carrying out individualized plans.
  • Works with Behavior Analyst, RN, and Manager to ensure team members are prepared for emergency placements and transitions into Individualized Supported Living (ISLs). Communicates proactively to other organizational staff regarding transitions, including nurses and behavior team members.
  • Develops daily schedule of activities with and for individuals served and ensures follow-through; communicates to support team members regarding observations and interventions provided as it relates to individuals' functioning, health status or behavior; and ensures that homes provide a healthy and safe living environment for individuals served.
  • Interviews, hires, and trains new staff. Oversees onboarding activities, provides routine and regular coaching and feedback, and completes performance evaluations. Ensures staff remain current in their required trainings.
  • Schedules team members and monitors overtime. Ensures that homes operate in accordance with established staffing ratios and overtime guidelines.
  • Oversees and provides documentation for individuals served including tracking behavior and financial documentation. Organizes and reports data related to individual plans including monthly packets. Supervises the maintenance of all required staff and individual records. Ensures that documentation and homes meet all agency guidelines, funder requirements and accreditation standards.
  • Attends ISP meetings and provides input into planning process. Prepares for and attends service coordinator visits.
  • Attends nursing visits and is responsible for medication oversight. Reviews all medications and medical administration records. Reports any charting or medication errors according to departmental protocols. Attends doctor's appointments and provides training on adaptive equipment.
  • Attends meetings, in-services and opportunities for on-going education. Facilitates and participates in regular staff meetings and reviews any issues and training needs.
  • Works as fill-in direct care staff as needed.

Qualifications:

  • Must have high school diploma or equivalent. Completion of college-level coursework in the human services field preferred.
  • Must have at least 2 years of experience providing direct support to individuals with disabilities, including at least 1 year of experience providing direct support to individuals requiring a high level of behavioral support.
  • Must have at least one year of previous leadership experience working with individuals with disabilities or individuals with high-level behavioral support needs.
  • Must have knowledge of Microsoft Office products and be able to work with a variety of computer systems.

Additional Requirements:

  • This position may work day, evening, night and/or weekends depending on the needs of the organization. Provides 24-hour on-call support to individuals and employees, with rotating weekend on-call duties.
  • This position requires driving personal and/or company vehicle. Transports individuals to activities, appointments, and other community sites.
  • Must have reliable vehicle and maintain current driver's license and proof of insurance.
  • Must complete and maintain all required trainings/certifications.
  • Must maintain confidential information in accordance with HIPAA regulations.

Skills Required

  • High school diploma or equivalent
  • Completion of college-level coursework in human services
  • At least 2 years direct support experience with individuals with disabilities, including 1 year supporting high-level behavioral needs
  • At least 1 year prior leadership experience working with individuals with disabilities or high-level behavioral support needs
  • Knowledge of Microsoft Office and ability to use various computer systems
  • Ability to work varied shifts (day/evening/night/weekends) and provide 24-hour on-call support with rotating weekend duties
  • Must have reliable vehicle, current driver's license and proof of insurance; ability to transport individuals
  • Complete and maintain all required trainings and certifications
  • Maintain confidential information in accordance with HIPAA regulations
