Essential Functions:
- Provide direct supervision to Care Coordinators and Care Navigators and ensures alignment with The Center for Family Safety and Healing (TCFSH) protocols, policies, and best practices.
- Ensure timely and effective coordination of services, including referrals, follow-up, and documentation.
- Provide onboarding and ongoing training to staff, with an emphasis on trauma-informed care, safety planning, and resource navigation.
- Support staff in addressing complex client needs, including high-risk and crisis situations.
- Track program metrics and outcomes, supporting continuous quality improvement efforts.
- Support implementation of organizational initiatives.
- Identify trends, gaps, and opportunities to improve access and service delivery.
Education Requirement:
- Bachelor’s Degree in Social Work, Counselling, Public Health, or related field, required.
- Master’s Degree, preferred.
Licensure Requirement:
Licensure (e.g., LSW, LISW, LPC, LPCC), preferred.
Skills:
- Ability to develop and implement an integrated care coordination model across TCFSH programs.
- Ability to establish standardized workflows, referral pathways, and care management protocols.
- Strong verbal and written communication skills, with the ability to clearly convey information to team members, healthcare providers, and families in a professional and tactful manner.
- Ability to coach team members through change, provide constructive feedback and recognition, delegate tasks effectively, and make informed decisions.
- Ability to foster collaboration and build positive working relationships within the team.
- Ability to effectively interact with individuals from diverse age groups and backgrounds.
- Strong organizational skills with a high level of attention to detail.
- Ability to ensure services are trauma-informed, survivor-centered, culturally responsive, and equitable.
- Ability to promote continuous quality improvement and evidence-informed practices.
- Proficiency in Microsoft Office applications, including Outlook, Word, PowerPoint, Excel, and Teams.
Experience:
- Three years of experience in care coordination, case management, or a related human services field, required.
- Prior supervisory or team lead experience, preferred.
- Demonstrated experience working with individuals impacted by trauma, family violence, or complex social needs.
- Experience in healthcare, child welfare, domestic violence, or community-based settings.
- Experience with Electronic Health Record (EHR), preferred.

Skills Required
