We are currently seeking a dynamic individual to serve as the Complex Case Manager for Level 1 or 2 for the Lead Program. 

Please note that all applicants must undergo a pre-employment drug screening, which includes testing for marijuana, as well as a comprehensive background check and TB Test.

SUMMARY:

Law Enforcement Assisted Diversion (LEAD) is a harm reduction program that works to connect low-level substance users with social services to reduce future involvement with the criminal justice system and improve quality of life.

The LEAD Complex Case Manager (CCM) works directly with individuals who are struggling with substance use disorder (SUD), mental health issues, homelessness, and other challenges and provides stabilizing supports and services.  The CCM meets with LEAD participants and community referrals, conducts intakes and assessments, and connects participants with available resources. The CCM develops and maintains collaborative relationships with housing providers, service providers, referral sources, including law enforcement officers, and other community organizations.  

ESSENTIAL DUTIES AND RESPONSIBILITIES: Include the following. Other duties may be assigned.

  • Adhere to LEAD principles and policies.
  • Serve as a point of contact for LEAD referrals.
  • Respond to law enforcement for warm handoff according to LEAD policy.
  • Complete LEAD candidate intake and psycho-social assessment, as needed.  
  • Provide complex case management services to LEAD participants.
  • Develop individually tailored case management plans designed to meet each participant’s identified goals.
  • Develop and maintain client files according to agency and contract requirements.
  • Document all activities according to agency and contract requirements.
  • Document all program referrals in the case management database.
  • Update client data in the case management database.
  • Follow up with referral sources and document LEAD participant outcomes.
  • Provide street outreach to engage individuals who have been referred to LEAD.
  • Work with community-based referral sources to engage participants when needed.
  • Seek out and collaborate with innovative interventions for LEAD participants.
  • Deliver LEAD training to law enforcement and other agencies and community partners.
  • Provide LEAD information to potential referral sources
  • Develop and maintain cooperative relationships with Pueblo County Sheriff’s Office, Pueblo Police Department, 10th Judicial District Attorney, and others to coordinate services
  • Develop and maintain collaborative relationships with LEAD partners and other service providers, including chemical dependency treatment providers, mental health providers, health care providers, shelter providers, landlords, withdrawal management services, and DHS workers.
  • Attend all LEAD Program staff meetings and other required meetings.
  • Participate in Operational Workgroup meetings.
  • Participate in public information and community-awareness events.
  • Participate in scheduled training.
  • Shift work, nights, weekends, and holidays may be required.
  • On-call status may be required.
  • Additional duties as assigned.

Qualifications

QUALIFICATIONS:

Education:    

Must have a minimum of a high school diploma/GED.  Bachelor’s degree in a related field preferred. 

Must be able to earn the required certification within 18 months of hire.

Level 1     Must obtain required certification within 18 months.

Level 2     Must have a minimum of 3 years of Complex Case Management experience and obtain the required certification within 18 months.

Experience:  

At least two (2) years of experience in the behavioral health or human services field preferred.

Other:       

  • Ability to respectfully engage with people experiencing homelessness, substance use disorder, and mental health disorders.
  • Willingness to follow harm reduction principles.
  • Experience in data collection, case management, and clients struggling with SUD.
  • Experience developing safety plans.
  • Ability to work with diverse populations in various states of anxiety, sobriety, and aggression.
  • Ability to verbally de-escalate individuals.
  • Ability to visually assess for mental or physical health crisis.
  • Ability to visually assess for intoxication.
  • Ability to quickly problem-solve and make decisions.
  • Computer literate with basic knowledge of Microsoft Office Suite.
  • Must possess a valid driver's license and be insurable.

PHYSICAL DEMANDS:

Must be able to stand, walk, and sit (including driving) for extended periods of time. Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions.

WORK ENVIRONMENT:

Typically work standard office hours, but during busy periods, may be required to work additional hours, including evenings or weekends. Responsible for attending networking events, launches, and exhibitions, which usually happen outside office hours.
