The Community Support Specialist / Supportive Services for Veteran Families (SSVF) provides direct service to veterans and their families to address housing crises, connect to VA benefits, link to community resources, and help transition families to self-sufficiency. SSVF provides housing-focused case management, outreach, benefits navigation, and temporary financial assistance.
ESSENTIAL DUTIES & RESPONSIBILITIES
- Develops and reviews members’ care plans by identifying goals, objectives, and specific mental health services needed.
- Develops individualized Housing Stability Plans, including SMART goals.
- Assist with housing outreach, conduct housing inspections, and rent negotiations with landlords.
- Administer rental, utility, and other stabilizing assistance as allowable under SSVF guidelines.
- Teaches members independent and community living skills, such as housekeeping, meal preparation, coping skills, etc.
- Provides support to members with collateral social service agencies and community partners, including shelters and VA hospitals.
- Plan and facilitate various activity groups.
- Documents services provided in an Electronic Health Record in a timely manner.
- Other duties as assigned.
EDUCATION/EXPERIENCE:
- Candidates must be at least 21 years of age and have.
- Proficient in Microsoft Word and Excel.
- High school diploma or GED certificate with 5 years of supervised job-related experience in the field of social services OR Bachelor’s degree in Psychology, Social Work, or related field
- Housing experience such as working with CHA and Private Landlords preferred.
REQUIREMENTS
- Daily access to a personal vehicle
- Valid driver’s license.
- Military experience preferred.
- Willingness to provide transportation for members of the program.
- Must obtain and maintain liability car insurance of:
- $100,000 per person bodily injury.
- $300,000 per accident.
- $100,000 property damage.

- Competitive Pay - Base Rate: $21.75 - $25.87 hourly / $45,240 - $53,800 annually
