Provide and coordinate community-based rehabilitative and support services for consumers, develop and implement individualized habilitation plans, apply IMR principles, manage mental and physical health services, perform crisis intervention, maintain schedule and productivity (60% face-to-face), and operate agency vehicle for client visits.

Contributes to the development, implementation, monitoring, and updating of Consumers’ individualized habilitation plans; therapeutic rehabilitative skill development; implementation of Illness Management and Recovery (IMR) principles and interventions; coordinating and managing services, both mental health and physical health; crisis intervention, as needed.  Provision/coordination of services aimed at active rehabilitation and support services to achieve restoration of functioning and the achievement of community integration; maintaining own appointment schedule and meeting work productivity requirements.  Other duties as assigned by management staff.  Productivity requirement of 60% face to face contact weekly. Must be able to operate an Agency vehicle and maintain a driving record acceptable to Agency’s insurance provider.

Qualifications

Education/Experience:

Bachelor’s Degree from an accredited college or university in one of the helping professions, such as social work, counseling, psychiatric rehabilitation, or psychology. Bachelor’s Degree in a field other than a helping profession shall have a minimum of two years’ professional experience working in a community based behavioral health setting. Certified Psychiatric Rehabilitation Practitioner (CPRP) may be substituted for one year of experience.
