Position Summary
The Community Strategist will be supporting Non-Medical Drivers of Health (NMDoH) Medicaid Product Function in terms of market launching the Community CARES Team. Non-Medical Drivers of Health include factors like socioeconomic status, education, neighborhood and physical environment, employment, and social support networks, as well as access to health care.
 

This role will learn and understand available community resources and programs; utilize data and local knowledge to build grassroots, strategic, and data-informed community partnerships; and identify opportunities to develop and pilot new NMDoH initiatives. 

Job Duties 

  • Implement and scale the overall strategy to address health equity & NMDoH for members and communities in assigned region.
  • Operationalize NMDoH product in assigned region.
  • Build partnerships to incorporate health equity solutions into NMDoH, population health, care management, quality, regulatory, and community outreach programs. 
  • Establish outcomes-based contracts with community-based organizations.
  • Organize community activation meetings to successfully execute key health equity & NMDoH initiatives with community partners.
  • Create and support business development initiatives to align with health equity & NMDoH strategy.
  • Cultivate and foster relationships with internal and external stakeholders to drive collective member impact.
  • Monitor and track health equity & NMDoH data and deliverables.
  • Serve as a public speaker at coalitions and conferences, in order positively position the organization.

Required Qualifications 

  • 3 + years of experience with public health, health equity, population health, care coordination, and/or community-based programs. 
  • Must reside in Texas, specifically in the Dallas Fort-Worth or San Antonio areas.
  • Ability to work Monday-Friday from 8am-5pm Central Time, as well as to have flexibility beyond those core hours.
  • Must possess reliable transportation and be willing and able to travel in-state up to 60-70% of the time. Please note that mileage is reimbursed per our company expense reimbursement policy. 

Preferred Qualifications 

  • Previous experience with building programming and/or submitting proposals for community programs.
  • Prior experience with managed care. 
  • Excellent interpersonal and strong communication skills.
  • Demonstrates good oral and written presentation skills.
  • Strong organizational skills, including the ability to prioritize, multi-task, and manage workload to meet specific timeframes and deadlines.

Education

  • Bachelor’s Degree in health administration, social services, public health, business administration, psychology, or closely related field.

Pay Range

The typical pay range for this role is:

$46,988.00 - $102,000.00

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.  The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.  This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
