The Community Liaison supports patients transitioning from hospital-based care to home infusion services. This highly patient-facing role works closely with patients, care managers, and hospital teams at the bedside to educate them on the Home Infusion model and help coordinate a smooth transition home. The caregiver also serves as a subject matter expert on home infusion equipment, providing education and troubleshooting support to patients and clinical partners while building strong relationships with healthcare stakeholders.
A caregiver in this position will work Monday through Friday, 9:00am to 5:00pm. Fairview, Avon and Lutheran will be the floating locations for this role. No weekends, holidays or on-call required.
A caregiver who excels in this role will:
- Identifies appropriate referral sources and builds positive business relationships.
- Provides education about Cleveland Clinic homecare services to the referral source and to patients.
- Meets / exceeds established referral goals.
- Facilitates accurate and timely completion of required documentation.
- Serves as the primary contact for the referral source. Ensures a positive ongoing relationship between the referral source and Cleveland Clinic homecare.
- Analyzes referral source data. Monitors the local market and the competition.
- Recommends marketing initiatives based on trend analysis and enterprise / department strategy. Works with the marketing team to develop collateral.
Minimum qualifications for the ideal future caregiver include:
- High School diploma or GED
- Valid Ohio Driver’s License
- Proof of Automobile Insurance coverage of $100,000/$300,000
Preferred qualifications for the ideal future caregiver include:
- Bachelor’s degree in Business, Marketing, Communications, Health Education or related field
- Previous healthcare or marketing experience
Physical Requirements:
- Manual dexterity to operate office equipment.
- May require periods of sitting, standing, and the ability to walk to various locations throughout the community to attend meetings.
- Must have normal or corrected vision to clearly communicate verbally by phone or in person.
- Must be able to lift at least 20 pounds.
Personal Protective Equipment:
- Follows standard precautions using personal protective equipment as required.
