As a Field Based Community Health Worker (CHW), you will act in a liaison role with Medicaid members to ensure appropriate care is accessed as well as to provide home and social assessments and member education. CHW ' s work in a team - based structure and spend the majority of their time in the community engaging directly with members. Interest in learning to work with medically complex patients who may be experiencing significant addiction and/or behavioral health conditions is important for this role.
This position is full - time (40 hours / week) Monday - Friday 8am to 5pm. It may be necessary, given the business need, to work occasional overtime. This position is a field - based position with a home - based office. You will work from home when not in the field.
If you reside in the Western Region of Kentucky, you will have the flexibility to work remotely* as you take on some tough challenges.
Local travel up to 60% and mileage is reimbursed at current government rate.
Primary Responsibilities:
  • Follow up with members to monitor and document completion of plan activities following standard protocols
  • Monitor progress toward plan goals through evaluation of the effectiveness of programs and/or services provided
  • Make changes to plans or recommendations based on evaluation outcomes
  • Educate members/caregivers in self-management and plan benefits
  • Provide individualized education on preventative healthcare measures (e.g., immunization, healthcare screening)
  • Provide guidance or additional assistance to facilitate execution of plans or recommendations to improve member health
  • Educate members on health and wellness information related to disease processes and plan benefits
  • Educate member on appropriate utilization of services based on need (e.g., PCP vs. ER)
  • Work with internal partners to create a culture of health and wellness that promotes member health
  • Plan and execute on-site health and wellness programming for members and engage members in available programs
  • Use Technology to obtain and document Health-Related Information
  • Research member-related health information (e.g., medications, clinical records, policies, benefit information) using internal and external systems
  • Use desk top computer applications (e.g., Excel, Word, Outlook, Office Communicator) to summarize information (e.g., reports) or communicate member information to others.
  • Document information (e.g., assessment information, member interactions, referrals, follow up plans) in relevant computer systems.
  • Navigate multiple applications at once to obtain and/or document member information
  • Learn updates and changes to computer systems/applications and apply to navigate applications effectively
  • Execute and meet performance goals within a virtual environment
  • Navigate Virtual Call Center (VCC) system to receive inbound calls, make outbound calls, and document call time and activity appropriately

Required Qualifications:
  • High School Diploma / GED
  • Lived in the local community for 2+ years
  • Must have a designated workspace inside the home with access to high - speed internet availability
  • Intermediate level of experience navigating a PC to open applications, send emails, and conduct data entry
  • Intermediate level of knowledge of culture and values of the community with familiarity with the resources available in the community
  • Reside in Kentucky
  • Ability to travel locally up to 60% of the time

Preferred Qualifications:
  • Bachelor's degree in social work, licensed practical nurse (LPN), CNA/HHA or HS graduate with 3 years of experience working within the community health setting in a health care role.
  • Knowledge and / or experience with behavioral health or substance use disorders
  • 2+ years of field - based experience
  • 2+ years working in a case management role
  • Previous care management experience
  • CHW Certification
  • Experience working in Managed Care
  • Knowledge of Medicaid and / or Medicare population

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $20.00 to $36.00 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
