Job Summary:

Join our team as a Service Facilitator and Care Coordinator for youth and young adults enrolled in the PROPs First Episode Psychosis program. PROPs provides services primarily under the Dane County Comprehensive Community Services (CCS) system of care, which offers an array of mental health and substance use services in a team of providers of the participant’s choosing.

Duties/Responsibilities:

  1. Coordinate care for 10-15 youth and young adults experiencing their first episode of psychosis.
  2. Complete intake process, interviewing consumer and family members to build the comprehensive assessment, recovery plan, and provider team to support the individual.
  3. Coordinate and facilitate consumer Recovery Team Meetings at a minimum of every six months to update recovery plan and continually assess treatment goals.
  4. Utilize effective strategies to engage program participants, family members, and natural supports to enhance motivation and encourage meaningful, collaborative, and strengths-based goal planning and progress.
  5. Maintain regular contact with program participants to a degree sufficient to support effective engagement to assist with making and evaluating progress.
  6. Provide or obtain support and crisis/emergency services for consumers as appropriate.
  7. Monitor the provision and quality of services provided and act as the liaison when new services/resources need to be sought or developed.
Qualifications

Education and Experience:

Education/Licensure

ESSENTIAL

  • Bachelor’s degree in a behavioral health or related field
  • Possession of a valid driver’s license, a favorable driving record, adequate insurance (per state law), and access to a vehicle. 

 

PREFERRED

  • Master’s degree in a behavioral health or related field
  • Bilingual (must be able to pass Alta Language Proficiency Test)

 

Experience

PREFERRED

  • Experience with use of evidence-based practices in behavioral health.
  • Commitment to work toward improving cultural diversity and inclusion.
  • Commitment to principles of recovery and to facilitating consumer participation in treatment/service planning and provision.
  • Experience in community-based behavioral health team.

 

Physical Requirements:

  • Requires driving, sitting, standing, light lifting, bending, and reaching; and the ability to use and operate standard office equipment, including computer, calculator, phone, printer, cell phone, facsimile, etc.

 

Working Conditions

  • In person or limited hybrid

 

Hiring Process:

  • All potential candidates will need to successfully pass the Background Information Disclosure (BID) for entity employees and contractors.  Wisconsin’s Caregivers Law requires background, and criminal checks are required by the Department of Health Services and Wis. Stat. 50.065(6)(c) and Wis. Admin Code § DHS 12.05(4)
