- Document client encounters from outreach events, activities, service plans, and outcomes achieved by client in an effective manner; completes and submits monthly reports; and ensures accurate data collection for quality measures and other reporting purpose
- Coach patients in effective management of their chronic health conditions and self-care. Assist patient in understanding care plans and instructions. Motivate patients/clients to be active and engage event participants in their health and overall wellbeing.
- Assist patients in accessing health-related services to appropriate medical and specialty providers and overcoming barriers to obtaining needed medical care and /or social service
- Comply with Health West Inc.s policies and procedures

Education
High School Diploma or Equivalent
Graduation from a Medical Assistant Program or Equivalent
Graduation from a Practical Nursing Program or Equivalent
Experience
One year recent experience in a health care setting preferred. Current National Medical Assistant Certification required. Current Idaho Practical Nursing License or equivalent required. Current certification in BLS.
