The Role
Provides telephone triage, patient follow-up, medication refills, referral and recall tracking, care coordination, and clinical documentation. Floats to in-person nursing care, supports outreach and quality improvement, coaches patients on chronic condition management, and helps patients access medical and social services. Maintains accurate records, reports, and compliance with clinic policies.

  2. Perform patient follow-up and coordinates care (e.g. obtain prior authorizations, follow-up on abnormal results, manage appropriate buckets in EMR, etc.), per clinic protocol
  3. Refill patient medications per clinic protocol

  12. Coach patients in effective management of their chronic health conditions and self-care. Assist patient in understanding care plans and instructions. Motivate patients/clients to be active and engage event participants in their health and overall wellbeing.
  13. Assist patients in accessing health-related services to appropriate medical and specialty providers and overcoming barriers to obtaining needed medical care and /or social service
  14. Comply with Health West Inc.s policies and procedures
  15. Other duties as assigned
Qualifications

Education

High School Diploma or Equivalent

Graduation from a Medical Assistant Program or Equivalent

Graduation from a Practical Nursing Program or Equivalent

Experience

One year recent experience in a health care setting preferred. Current National Medical Assistant Certification required. Current Idaho Practical Nursing License or equivalent required. Current certification in BLS.

Skills Required

  • High school diploma or equivalent
  • Graduation from a Medical Assistant program or equivalent
  • Graduation from a Practical Nursing program or equivalent
  • One year of recent experience in a healthcare setting
  • Current National Medical Assistant Certification
  • Current Idaho Practical Nursing License or equivalent
  • Current certification in Basic Life Support (BLS)
