Role Summary

The Clinical Pharmacist, Chronic Condition Management is an integral member of the primary care team responsible for optimizing medication therapy, improving chronic disease outcomes, and supporting the management of high-risk, medically complex patients. This position provides direct patient care, supports population health initiatives, and operates under collaborative practice agreements (CPAs) where permissible.

Key Responsibilities

Direct Patient Care

  • Provide comprehensive medication management (CMM) for patients with chronic conditions (e.g., diabetes, hypertension, heart failure, COPD, asthma, hyperlipidemia, and anticoagulation).

  • Conduct telehealth (or face-to-face, as applicable) visits to assess medication efficacy, safety, adherence, and patient understanding.

  • Develop individualized care plans in collaboration with an interdisciplinary care team (physicians, advanced practice providers, nurses, care coordinators).

  • Provide patient education on disease states, medications, and self-management strategies.

  • Ensure drug therapy appropriateness, dosing correctness based on patient-specific parameters, and cost-effective medication selection.

  • Interpret laboratory and clinical data to develop treatment plans and monitor therapeutic response.

  • Deliver evidence-based recommendations to the managing medical team regarding alternative approaches and regimens.

  • Manage patients under collaborative practice agreements, including initiating, adjusting, and discontinuing medications where permissible.

Clinical Program Development & Population Health

  • Identify and engage high-risk or high-cost patients using population health tools, risk stratification reports, and EMR dashboards.

  • Participate in quality improvement initiatives focused on chronic disease metrics (e.g., A1c control, blood pressure goals, medication adherence).

  • Collaborate with care teams to reduce hospital readmissions and emergency department utilization.

  • Develop and maintain clinical protocols, workflows, and documentation templates to support pharmacist-led services.

  • Track and report clinical outcomes, intervention metrics, and quality measures.

Interdisciplinary Collaboration

  • Serve as the medication expert on the primary care team, offering evidence-based recommendations for complex regimens.

  • Communicate clinical interventions and care plans effectively via EMR and team huddles.

  • Participate in care team meetings and provider education sessions.

  • Provide clear, succinct, and professional verbal and written recommendations.

  • Ensure a continuum of care by cooperating with external healthcare professionals and organizations.

Qualifications

Required:

  • Bachelor of Science in Pharmacy or Pharm.D. degree.

  • Current and unrestricted Pharmacist license in state of practice (obtainable within 60 days of hire), with the ability to obtain licensure in additional states as needed.

  • Completion of a PGY1 pharmacy residency or 3+ years of clinically relevant experience.

  • Ability to obtain Board Certification (BCACP, BCPS, BCGP) within one year of eligibility.

  • Legal authorization to work in the United States.

  • Ability to travel up to 20% of the time based on business needs.

Preferred:

  • 2+ years of experience in an ambulatory care, primary care, or population health setting.

  • Active Board Certification (BCACP, BCPS, or BCGP).

  • Completion of a PGY2 pharmacy residency.

Pay Range

The typical pay range for this role is:

$118,000.00 - $187,500.00

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.  The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.  This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
