Job Summary
The Clinical Pharmacist is responsible for the provision of clinical pharmacy services including: anticoagulation, pharmacokinetics, antibiotic stewardship program, drug dosing consultation, nutritional support monitoring, drug information, formulary reviews, in-services on pharmacology/disease state pharmacotherapy, pharmacokinetics to medical, nursing, and pharmacy staff. This includes the selection of medications to disease state, monitoring of patient’s therapeutic outcomes, follow-up on medication errors and evaluation of adverse drug reactions. Provides prescribed medications, other pharmaceuticals and compounded medications according to professional standards and practices, hospital policies and procedures, regulatory agencies, and Federal and State laws. Performs medication order verification, supervises preparation of all sterile IV medications, narcotic dispensing and accounting, dispensing medications. In addition, may perform drug use evaluation, quality assurance, maintain the formulary, and review/report adverse drug events.
Qualifications

Required
- Current California Registered Pharmacist License
- A Doctor of Pharmacy degree from an accredited school of pharmacy
- Completion of ASHP-accredited PGY1 residency training in an acute care setting
Preferred
- Board Certified Pharmacotherapy Specialist (BCPS)
- Minimum of 1 year of acute care hospital experience
