What You'll DoAs a Clinical Pharmacist, you will:
- Review and evaluate medication orders to ensure safe and appropriate drug therapy.
- Assess patient medication regimens and optimize treatment outcomes.
- Collaborate with physicians, nurses, and other healthcare professionals to provide comprehensive patient care.
- Identify, prevent, and resolve medication-related issues, including drug interactions, contraindications, and adverse effects.
- Provide medication counseling and education to patients and caregivers.
- Serve as a clinical drug information resource for healthcare teams.
- Monitor patient responses to therapy and recommend adjustments when appropriate.
- Participate in medication utilization evaluations, adverse drug reaction reporting, and quality improvement initiatives.
- Support pharmacy operations while maintaining compliance with regulatory, accreditation, and safety standards.
- Assist in the education and precepting of pharmacy students and residents.
- Present educational programs and in-services to clinical staff as needed.
- Bachelor of Science (BS) in Pharmacy or Doctor of Pharmacy (PharmD) from an accredited school of pharmacy.
- Current pharmacist license in the state of practice.
- Strong clinical judgment and commitment to patient safety.
- Ability to work effectively in a fast-paced healthcare environment.
- Two or more years of clinical pharmacy experience.
- Completion of a PGY-1 Pharmacy Residency.
- Experience in a hospital or health system setting.
- Familiarity with Epic, Micromedex, Omnicell, McKesson Automation, ScriptPro, or similar pharmacy technologies.
