Midland Care Connection is a not-for-profit, community-based organization in Topeka, Kansas, dedicated to helping older adults and seriously ill individuals stay independent and supported in their homes. Established in 1978, we offer a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and our signature PACE (Program of All-Inclusive Care for the Elderly) program. Our mission centers on "meeting individual needs through innovative care solutions," and they serve people across multiple counties in northeast Kansas.
Visting Nurse Association
Specializes in home hospice that provides compassionate, end-of-life care to patients in the comfort of their own homes. Their skilled nurses focus on managing pain and symptoms, supporting emotional and spiritual needs, and guiding both patients and families through the hospice journey. Working closely with an interdisciplinary team, the VNA ensures dignity, comfort, and quality of life during a patient’s final days while offering education, resources, and respite support to caregivers.
Clinical Liaison
Serves as a link between the organization and healthcare providers, patients, and families by promoting VNA’s services and helping eligible patients access care. The role focuses on building and maintaining relationships with referral sources such as hospitals, clinics, nursing facilities, and care coordinators, while educating them about hospice, home health, and other supportive services. Clinical liaisons assist with referrals, support parts of the admissions process, and help ensure smooth transitions of care, often meeting with patients and families to explain services and answer questions. They also participate in community outreach and educational efforts, represent VNA at events, and track referral activity, all while maintaining professionalism, confidentiality, and alignment with the organization’s mission.
QualificationsRequired
- Clinical background (such as RN, LPN, social work, or related healthcare experience)
- Knowledge of hospice, home health, palliative care, or community-based healthcare services
- Strong communication and interpersonal skills, with the ability to speak comfortably with providers, patients, and families
- Ability to build and maintain professional relationships with referral sources
- Organizational skills and attention to detail for managing referrals and documentation
- Valid driver’s license and ability to travel locally to hospitals, clinics, and community sites
- Commitment to confidentiality and HIPAA compliance
Preferred
- Previous experience in healthcare marketing, outreach, admissions, or care coordination
- Familiarity with discharge planning processes in hospitals or skilled nursing facilities
- Experience working with end-of-life care or seriously ill populations
- Knowledge of Medicare, Medicaid, and hospice eligibility criteria
- Strong presentation and community education skills
- Experience using electronic medical records (EMR) or referral tracking systems
