Position Summary

The Clinical Liaison serves as the first point of contact for hospitalized patients and their families who may benefit from Ennoble Care’s hospice and palliative care services. This role is responsible for educating patients, families, and healthcare partners about Ennoble Care’s programs, supporting informed decision-making, and facilitating smooth transitions of care. The Clinical Liaison collaborates closely with physicians, interdisciplinary team members, and acute care partners to develop and initiate individualized, goal-aligned care plans.

Key Responsibilities
  • Serve as the primary liaison between Ennoble Care and acute care facilities, identifying patients appropriate for hospice or palliative care services.
  • Introduce and educate patients and families on Ennoble Care’s hospice and palliative care philosophy, services, and benefits.
  • Provide emotional support and guidance to patients and families during care transitions.
  • Collaborate with patients, families, physicians, and the interdisciplinary team to develop care plans aligned with patient goals and wishes.
  • Conduct nursing assessments and make clinical judgments based on patient needs and eligibility criteria.
  • Initiate care plans and coordinate discharge planning in collaboration with Ennoble Care’s hospice and palliative care medical consultants.
  • Communicate patient goals, clinical status, and treatment plans effectively with interdisciplinary team members.
  • Promote Ennoble Care’s hospice and palliative care programs within hospitals, skilled nursing facilities, and other referral sources.
  • Build and maintain strong relationships with facility leadership and staff through education, in-service trainings, and community outreach.
  • Rotate to other regions or departments as business needs require.
Qualifications & Requirements
  • Minimum of one (1) year of experience in a comparable clinical setting or unit.
  • Minimum of one (1) year hospice and/or marketer experience. 
  • Needs hospice and/or palliative care experience.
  • Graduate of an accredited nursing program as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) required.
  • BSN preferred for RN candidates.
  • Current nursing licensure as an RN or LPN in the applicable state(s) of practice (compact state licensure accepted where applicable).
  • Valid driver’s license in good standing with access to a reliable vehicle during working hours.

Base Salary: $50,000 - $95,000, Plus: Monthly, Quarterly, and Annual Bonuses

Skills Required
